HC Telehealth is a virtual clinic. The clinic may choose to do a video consult or telephone consult pending the complaint. You understand HC Telehealth is NOT for emergencies. If this is an emergency, you should call 911 or go to an emergency department. You understand that by using this telehealth service you are seeking care that is convenient and affordable. Using telehealth, you are giving the correct information, pictures and health questions regarding themselves is true and honest. You understand that by using this service you will not have an in-person office visit for a physical exam that might identify a condition that needs further investigation or treatment. You agree to the risks and benefits associated with the visit. You agree to read the information that is provided with your medications from the pharmacy regarding risks and benefits. You understand that all prescriptions advised such as over the counter or prescribed can cause serious side effects and adverse events that include allergic reaction, permanent disability, and death. You agree in an adverse event including signs and symptoms of serious side effects from taking medications you will seek appropriate medical care in a timely manner. You have the right to outweigh the risks and given the alternative option available which is not using telehealth for your care. You will need to continue with your normal physical exams with your primary care physician for routine care and chronic conditions.
