Transitions Health provides medical care services to residents at the HCMC, patients at Plumley Rehabilitation Center, and recently discharged patients who might have trouble accessing primary care immediately following discharge from the hospital. Transitions Health will be able to provide a number of services which include, but are not limited to:

- Hands-on care and monitoring to prevent complications and hospitalizations
- Conducting physical exams
- Assisting with medication treatment
- Ordering lab work, screenings and tests
- Providing respiratory treatments
- Ordering IV antibiotic therapy
- Applying and Removing Sutures and staples
- Diagnosing and treating acute and chronic diseases
- Prescribing medication and therapy

Materials Management had one on vacation & another out sick and Diana was willing to step in & cover stocking the medical floors during the week. We are a little behind on our recognitions, but we have some outstanding recipients. This , we want to congratulate Denise Brogdon, HCMC Hospice. During Christmas, Denise had several admissions all at once. She never complained and she always looks for ways to make admissions happen. Her motto is to get them admitted ASAP and always get them admitted on the same day of referral or as soon as they come home from the hospital or SNF.

With it being Nurse Practitioners week I just want to say thank you so much to Tammy Holcomb for all that she does. Not only is she brilliant and thorough with my health care, she is also kind and compassionate in all that she does. She listens and takes time to hear your concerns and never treats you like a number in the waiting room. I'm so thankful for her!
