As National Nursing Home Week came to a close, we are reflecting on the beautiful week of connection and gratitude. To wind down, we had two amazing visitors Ms. Laura Brown and Ms. Betty Franks visiting Patriot Health and Rehabilitation, and we treated our hardworking staff to an appreciation lunch. Our amazing residents returned the love by presenting their gifts!

Every day, our team goes above and beyond to provide compassionate care to those who call our community home. Seeing our residents celebrate the staff who care for them perfectly captures the family-like bond we share at Patriot.

Thank you to everyone who made this week so special. A special thank you to the community that dropped off special treats for residents and staff to enjoy for the week. Here is to continuing to support and uplift one another!

We are thrilled to highlight a wonderful example of leadership and employee engagement here at Patriot Health and Rehab!

To kick off this yearā€™s National Skilled Nursing Care Week, our Administrator, Charles Moore, brought resident and staff appreciation to life in the most memorable way by sitting down at the piano and performing for our community.

And the fun didnā€™t stop thereā€¦

Step right up for a spectacular carnival experience led by our Activities Director, Rebecca Hase! Residents and staff enjoyed everything from classic games and prizes to sweet treats and nonstop laughter.

Ready, setā€¦ ROLL! Who says racing only belongs on the track?

This week at Patriot Health & Rehab, we celebrated Nursing Home Week with the theme ā€œOur Care for Residents is Always in Full Bloomā€ and each day brought its own special meaning:

Monday shined bright in yellow, symbolizing the sunshine, happiness, and joy our residents bring into our lives.

Tuesday was all about red, representing the deep love we have for those we care for.

Wednesday bloomed with bright pastels and florals for ā€œBlooming Brights!ā€

Thursday embraced lavender for ā€œFlower Power,ā€ while also honoring Alzheimerā€™s and Dementia awareness. A special highlight was Ms. Karen Claxton, LPN, escorting residents from the dementia hall to enjoy the wheelchair race.

Friday wrapped things up in pink with a ā€œFloral Garden Party,ā€ celebrating compassion, care, and the sweet memories we share.

And the main event? Our wheelchair race had everyone smiling. One of our residents kicked things off with an enthusiastic ā€œOn your mark, get set, go!ā€ while others cheered from the sidelines.

From laughter to friendly competition, this week was full of the moments that make Patriot such a special place. Huge shoutout to our residents and staff for bringing the energy and heart all week long!

We are beyond proud to celebrate Grace Lankford, who has been named Nurse of the Year for Champion Careā€™s Tennessee region!

Grace is an extraordinary nurse whose compassion, resilience and expertise have transformed our community at Patriot Health & Rehab. Grace was selected for consistently going above and beyond in resident care, leadership as a mentor, and for her community dedication. Her expertise makes a massive difference, not only to our residents but to all of us who work alongside her. This recognition is a testament to her dedication, exceptional clinical skills, and profound compassion for the residents here at Patriot Health & Rehabilitation Center. Graceā€™s ability to go above and beyond is an inspiration to our entire team.

We are also extremely proud to announce many of our nurses received additional awards as we wrapped up Nurses Week!

Growth Guardian Award ā€“ (Karen Claxton - LPN) ā€“ Launched the Music & Memory program for Alzheimerā€™s and Dementia Patients at Patriot Health and Rehabilitation, which brings residents favorite music to seniors with dementia and other types of physical and cognitive limitations to enhance their memories and enrich their lives. Ms. Claxton shows true dedication daily to the residence!

Best Organizer ā€“ (Cindy Walton - LPN) for maintaining order during chaotic shifts.

Best Calm Under Pressure (Nicole Holmes -RN) for bring the ā€œCool as a Cucumberā€ nurse who stays composed and provides expert care in high-stakes situations.

Best Attention to Detail (Kris Gavin ā€“ RN) Consistently notices small details, ensuring accuracy in charting, medication administration and reducing risks to improve quality care.

Best Clinical Skills (Jessica Owens - LPN) the ā€œIV Queenā€ a nurse with exceptional technical skills and high-quality patient care.

Best Educator (Jonathan Kemble - RN) excellent in mentoring new grads, training peers, and fostering a strong learning environment.
