Full Job Description:

**Currently we are looking for a Visiting Registered Nurse for visiting patients in Park Ridge, Skokie and surrounding Chicago Cities **
*** IF WILLING TO SEE COVID-19 POSITIVE PATIENTS.*** Visits would be paid at a HIGHER RATE of $150 / Admission and $100 / Follow up Visit. Visits could total a Full time case load. (25-30 visits per week).
Responsibilities Include:
- Conduct Comprehensive Assessments (using the OASIS tool), develop and implement Plan of Treatment.
- Collaborate with Physicians, Agency staff and contract services to coordinate patient care.
- Initiate appropriate referrals for other medical services when indicated.
- Perform various types of Skilled Nursing visits.
- Promote and maintain patient health through teaching and learning appropriate self-care techniques.
- Document direct patient services in a timely manner in Electronic medical records.
- Collaborating with management team and office staff to facilitate timely office processing of paperwork and patient information.
- Actively participates in agency Quality Improvement activities.
- Maintain the highest level of Ethical Standards.
Qualifications:
- Must be a Registered Nurse (RN) with the Illinois Department of Professional Regulation.
- Must have a Minimum 2 years of Home Health Experience
- Psychiatric Nursing experience a major advantage
- Current CPR Certification
- Must have and maintain a valid IL Driver’s License, automobile insurance coverage
- Must be willing to travel to surrounding areas and patients homes
- Ability to work independently with minimal supervision and time management abilities
- Ability to successfully complete required background check
- Excellent communication and reporting skills
