The Case Manager will work directly with patients suffering from Substance Use Disorder in our Residential Treatment Program. The position will also involve patient families and outside legal, social, and medical services. The successful candidate will assist the patient in engaging appropriate resources and achieve coordination of care for a client after they are discharged from residential care. The Case Manager is a vital part of the Multidisciplinary Treatment Team. The Case Manager will schedule and coordinate weekly Team Meetings for our patients.

The Case Manager will:

Work with the client to create a service or recovery plan
Match the needs with available community services
Assist patients to access services
Use evidence-based or best practices
Live up to professional and ethical responsibilities
Complete necessary administrative tasks
Advocate for the client with a variety of service providers
Educational Requirement:

Required Credentials:
*Possess a Bachelor's Degree or Master's Degree in an approved Human Service Related Field (i.e. Psychology, Social Work, Sociology, etc.)
*WV License preferred but not required (ie. LPC, LGSW, LICSW, LCSW, LSW).
*Two years’ experience in treating alcohol and substance abuse (and/or AADC Certification) is preferred.

We will continue to serve the residents of West Virginia with quality substance use disorder and mental health treatment at our Summersville office, four mobile clinics,  and our Parkersburg location. We will continue to operate at our current location during the remaining 3 years of our lease and moving forward if the opportunity presents. JOB SUMMARY: The Behavioral Health Technician works is responsible for monitoring and reporting of patient behaviors, following daily scheduled patient activities, basic behavior management and intervention, assisting in daily care, crisis situations, and medical emergencies as needed as an integral part of the multidisciplinary team.

ESSENTIAL FUNCTIONS: • Maintains a constant awareness and supervision of patients including during attending gym/yard activities, crafts/art/group/recreation therapies and outing trips as assigned. Remains alert and aware of potential safety issues at all times. Alleviates any safety issue discovered by self, if appropriate, or by notifying appropriate persons for assistance as necessary. • Follows St. Joseph Recovery Center policies and procedures including personnel, nursing, and administrative policies. Maintains patient confidentiality at all times. • Serves as a positive role model for patients and peers providing examples of appropriate behaviors, effective communication, and conflict resolution. Encourages open, honest communication to facilitate problem solving on the unit. Charts daily in patient records. Individual is self-motivating and assists with unit cleaning, organization of new admits charts, filing, etc. Initiates patient/employee incident reports. • Provides a verbal report/briefing to the oncoming shift. Obtains and records vital signs and documents all care as required by company policy and in compliance with state and federal regulations. • Contributes to the patient assessment and care plan. Actively participates in unit therapeutic activities according to program schedules. • Assists with training of peers as assigned. • Follows infection control policies and assists with infection control rounds as assigned. • Other duties as assigned.

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

Suggest approaches that encourage patients to participate in wellness and recovery activities at a pace that is comfortable for them.

Background Experience:

Exceptional communication, organizational and interpersonal skills, with attention to detail required.
