They are the Before and After of a real patient with a real problem that plagues too many real people out there.

This is a new patient, basketball player, in his mid 20s. 🏀

Restored ankle = he can aggressively attack the rim and truly plantar flex and jump off his L ankle.

Limited ankle = he adopts a new game or he has a permanent intermission (retires) from it.

Humans are faced with this decision every single day. It’s our job as competent professionals to help them back to the activity they love and let them walk away on THEIR terms.

This results are standard operating procedure from my E&E course.

Represented 6 months to the day since I added “teach a seminar” to my journey. It also had a lot of memorable moments.

I had my first repeat course takers:
performance.systems attended the live course for the second time as he wanted to fine tune and dial in his manual skills. I’m pretty confident he manage an ankle better than 90% of the practitioners out there now.

took the digital course while in Australia 🇦🇺, but wanted to experience the hands on while in America.

graduated from PT school 1 week ago, and starts his post graduate work with my course. I am completely honored.

came in and leveled-up his game, as did - to have post-graduate trained clinicians in the room was quite the delight!

Emily from Denver, CO came in with her 20 years of experience and osteopathic exposure and asked some really great questions.

My dude came down looking for answers for his clients and I think he found some!
_jmorept from the crew came down and will immediately apply this to PM’s stack of NFL and NBA clients!

All in all, I had a lot of fun and we had a great case study at the end. It’s a lot of material for 8 hours but everyone did great and got a chance to level up their brains and hands in the interest of restoring knees, ankles and feet!

The Body Revisited

Just finished a 2-day cadaver dissection lab from

First off, I can’t recommend this course enough.

Second, I would like to thank the instructors Kyle and Erin for sharing their incredible knowledge base and skill.

I took the 2-day lumbo-pelvic module, but almost everyone else had been there for the entire dissection from Day 1 of the week long lab.

When I went through an embalmed cadaver 25 years ago at , I was trying my best to just memorize more than learn. Coming back to an unembalmed body with 23 years of clinical experience and perspective made this experience all the more memorable and fulfilling.

If you work with the body, and you work with other humans, I would now say this type of course is something you need to experience at least once in your career.

Absolutely Not

No, this new patient’s knees aren’t screwed.

There are plenty of new articular surfaces for him to use and “wear down”.

When you lose motion, you HAVE TO “live” and “wear and tear” on more focalized spots.

By restoring motion and the strength of the contractile tissues that are responsible for anti-gravity function, we can stave off that end-stage state.

These concepts, assessments and manual interventions are covered in my seminar “Efficient and Effective: Manual Therapy Assessment and Intervention for Restoration of Knee, Ankle and Foot Motion.”

PRI Symposium 2025

Last week I had the privilege, pleasure and honor of presenting at the Postural Restoration Institute, for my fellow PRI practitioners, and for all those who wear the “patient” hat.

My 20+ years in clinical practice with an orthopedic mindset has been enlightened and enhanced by the neurological lens and worldview that is not only PRI, but the Universe as well.

I don’t know and I’ll probably never know how or why of all the paths that life could have presented me, it gave me this one.

The presentations were incredible, I personally leveled up in so many different ways from them, and the scope of my awareness broadened because of them.

My presented was on how an ankle limitation might be the wrench in one’s PRI program, and how manual intervention might be the missing puzzle piece for that patient and practitioner.

I certainly don’t have all the answers, but I hope that my contribution helps someone out there.

Humbled and honored.

Japan 2025

The kids missed a few days of school 🤷🏻‍♂️.

Spent most of the trip in Osaka.

Last time we were mainly in Tokyo.

Next time it’ll be Kyoto or Sapporo.

Japan is living in the year 2100. I really do hope everyone gets a chance to experience it in their lifetime.

The Future is Here, Now

In the past I’ve been uncomfortable with things that felt “sales-y” or “shilly”. Not in this case. Not even close.

I am writing this with 100% confidence: the material in my E&E course, properly applied, will do a better job of restoring foot and ankle motion than anything else I’ve ever seen/done, because it was BUILT FROM SCRATCH.

This material will be in every professional, collegiate and academic setting. NFL, NBA, NCAA, DPT, etc., etc.

Assessments and Interventions were taken from anywhere and everywhere, including my clinical experience and opinion.

They were put through the fire with respect to clinical outcomes.

There are plenty of things I DON’T CARE about in my program that other sources do; the reason is simply: they don’t matter when it comes to patient outcomes.

The “proof” in terms of a study/published paper is still a ways away, but I’ll get to that when I can.

Who Am I? What Can I Do For You?

Hi, my name is Locatelli Rao and I’m a physical therapist and have been so since 2002. I specialize in the “complicated” cases that other providers have given up on.

My board certification is in Orthopedics but the reality is that just about everything is dictated by neurology.
