Cracking the Cone: Fitting Contact Lenses for Keratoconus
A review of patient candidacy, material and design trade-offs, advanced diagnostics and the expanding array of specialty treatments.

The ocular surface ecosystem plays an important role in contact lens design customization and wearability. Tear film instability, dry eye symptoms, lid/lash health and lid/globe congruity are just a few common examples that can affect contact lens comfort and fitting success among keratoconus patients. As keratoconus progresses without corneal stabilizing intervention, subsequent corneal nodule or scar formations, as well as post-surgical structural changes, may contribute other layers of fitting complexities that continue to hinder patient outcomes.

Reduced Renal Function a Risk Factor for DME
Diabetic patients with moderate-to-severe CKD are a high-risk group and may benefit from more frequent retinal screening exams for earlier detection.

Kidney transplants in endstage disease patients were associated with a reduced risk of DME, suggesting that improved renal function post-transplantation may lessen systemic microvascular complications, reducing the risk of retinal pathology.

Study Describes How Alzheimer’s and POAG Share Pathogenic Mechanisms

The researchers also asserted that RGC loss in glaucoma impacted brain function through a mechanism known as transneuronal degeneration.

Due to data source constraints and the cross-sectional nature of the study, the researchers were only able to compare Alzheimer’s with glaucoma. As a result, they could not determine the temporal sequence or causal relationship between Alzheimer’s and glaucoma.

The DEWS III report emphasizes the complex interplay between tear film abnormalities/instability, inflammation, increased osmolarity, neurosensory abnormalities and environmental factors. Distinctions between aqueous deficient disease and evaporative disease have been clarified, focusing on the role for metabolic, hormonal, physical, neural and cellular stressors.1 Inflammation and immune responses play a central role contributing to ocular surface damage and neuropathic pain that exacerbate symptoms and complicate management.
