October always brings back memories of the Harvard Laser Course — when I had the privilege of sitting among the true founders of laser medicine: Dr. Rox Anderson, Dr. Mathew Avram, Dr. Kilmer, Dr. Ross, Dr. Waibel, Dr. Carruthers, and many others.

“Hi Doc, I want subcision for my acne scars.”

That’s how many consultations begin.
Patients often say —

“I’ve tried Morpheus.”
“I did 12 sessions of microneedling.”
“I had Fraxel and CO₂.”
“Nothing worked. I want subcision.”

But here’s the truth: subcision is not a single procedure — it’s a category of techniques with huge variation.
To simplify, I divide this into three main differences:



1️⃣ The Scars

Every patient’s skin behaves differently.
Age, skin type, scar pattern, laxity, pigmentation, erythema, and medical history all play a role.
A 70-year-old with laxity and volume loss cannot be treated the same way as a 30-year-old with dense fibrotic scars.
Some scars are bound to ligaments; others are soft but sit on thin, delicate skin.



2️⃣ The Technique

Subcision can be done using needles, blunt cannulas, or mechanical liberators.
It can be performed with topical, local, or general anesthesia — each option changing the experience and risk.
Side effects vary from mild swelling to bruising or pigmentation changes.
At my practice, I often combine Ultraclear® or Fraxel non-ablative fractional lasers, PRP regenerative treatments, and biostimulators — addressing the surface, depth, and architecture in the same session for a balanced outcome.



3️⃣ The Physician

Every doctor approaches scar revision differently.
Some are comfortable with fully ablative lasers under anesthesia; others, like myself, prefer Subcision with Blunt Cannula, LaserCoring & regenerative combinations that minimize downtime and risk.
Experience and judgment determine results — not the device alone.



The Takeaway

It’s not as simple as “I want subcision.”
As a physician, planning scar restoration means balancing anatomy, safety, and long-term collagen rebuilding.
As a patient, understanding those differences is the first step toward results that last.✨

✨ Acne scars: years of trust, precision, and patience ✨

These photos show two different patients treated over several years with multiple modalities — subcision, RF microneedling, fractional lasers (Frax 1550 nm, CO₂, UltraClear laser-coring), platelet-rich plasma, exosomes, biostimulators such as Sculptra or Radiesse, and selective Botox.
Each plan was customized, often combining several procedures on the same day.

I consider it a privilege when patients trust me with this kind of long-term care. That trust goes both ways — just as they trust me with their face, I have to trust that they’ll follow every instruction carefully. Healing requires both sides working in sync.

I don’t treat “a scar”; I treat a person — their lifestyle, their downtime, their comfort, and their safety. I want to know if it’s realistic for them to return to work, if it’s safe to wear makeup, if they’re truly protecting their skin. These details determine outcomes as much as the treatment itself.

Some scars — especially on the forehead and temples — fall within high-risk vascular zones where filler injections can endanger vision. Every decision must balance improvement with safety and anatomical respect.

As a physician, I see not only the technical side but the emotional one — the courage it takes for patients to go through these procedures, tolerate the discomfort, and commit to aftercare. I feel deeply fortunate to guide them through that process.

Not everything that looks like filler actually is — and not every filler can be safely reversed. Here are six real stories that show why careful diagnosis and experience matter so much.

1️⃣ A patient who never had fillers — her natural anatomy created a hollow and bluish shadow that looked just like filler, proving that not all “filler problems” are filler related.

2️⃣ A patient with under-eye filler that turned bluish due to light reflection. It was a reversible hyaluronic acid filler, and with precise care, we were able to dissolve it and restore brightness.

3️⃣ A patient treated abroad with an unknown filler. It wasn’t fat or swelling — it was a poorly chosen product injected in the wrong plane. Thankfully, it was reversible, though several sessions were required.
⚠️ Procedures abroad can be cheaper, but they also carry serious risks — including blindness and skin necrosis when fillers enter a vessel. It’s never worth that risk.

4️⃣ A patient who underwent surgery for under-eye hollows and was left with a permanent complication. Even well-intentioned surgery can have lasting consequences if not done properly.

5️⃣ A patient who had fat transfer and filler in the same area. Separating where the fat ended and the filler began was extremely difficult — another reminder that “adding more” isn’t always better.

6️⃣ A patient whose reversible filler was corrected easily using an enzyme in the right hands — a perfect example of how science and skill can restore balance safely.



💬 The Takeaway

Every option — filler, fat transfer, or surgery — has pros and cons. The safest path begins with honest evaluation and restraint.
At my practice, nearly half of the people I see for filler consultations are advised not to have fillers at all. The goal is long-term harmony, not quick fixes.
