US Air Force Medical Service: Synensys has partnered with the Air Force to provide Patient Safety Specialist programming for 10+ years resulting in a 70% reduction in preventable patient harm events. Many of the 70+ medical treatment facilities have reduced serious safety events to zero as a result of the full implementation of system safety practices and high reliability principles. US Air Force Air Mobility Command: Air Mobility Command has been able to develop a customized Line Operations Safety Audit (LOSA) program capable of interfacing and integrating with multiple safety management data sets leading to comprehensive system improvement in safety culture, safety reporting, and threat and error management performance. Synensys successfully implemented new LOSA programming with Aeromedical Evacuation, Aviation Maintenance, and the Air Operations Center command

WellSpan Health: WellSpan developed a comprehensive system safety program for their largest hospital perioperative service line. Synensys conducted an organizational assessment of team-based safety practices, integrated change management tools and HRO principles, provided safety training for senior leaders, frontline trainers, and coaches, developed a system safety dashboard, and configured technologies to manage operational risk, improved quality of care, and increased patient safety. 100% of surgeons, nurses, residents, technicians, and anesthesia providers were trained despite COVID-19 surge operations.

By year nine, Synensys wasnโ€™t just improving safety - we were questioning the assumptions that held it back.

In 2013, Stephen M. Powell published a bold and insightful article identifying the Top 5 Dangerous Assumptions in Patient Safety. It called out the myths that slow progress, like believing communication is intuitive or that training alone can fix systemic issues. The message was clear. Safety demands systems thinking, not shortcuts.

This thought leadership sparked conversations across healthcare and defense sectors, encouraging leaders to rethink how they approach safety culture, teamwork, and accountability.

That same year, Synensys expanded its global reach, delivering TeamSTEPPS training to Aga Khan University Hospital in Dubai and deepening our work with defense clients on resilience management and human-centered design.

2013 was the year we stopped accepting assumptions and started reshaping the conversation.

By year eight, Synensys reached a new milestone in our mission to improve safety and teamwork across federal healthcare systems.

In 2012, we secured our first contract with the U.S. Department of Veterans Affairs, beginning work with VA Orlando and marking our entry into the Veterans Health Administration. It was more than a contract. It was a commitment to those who serve.

We partnered with IDEAS and Sector Health to deliver innovative training and performance improvement solutions tailored to the unique needs of veterans and their care teams. Together, we built programs that honored service with strategy, empathy, and excellence.

This collaboration laid the foundation for future VA partnerships and deepened our dedication to transforming care through connection.

By year seven, Synensys wasnโ€™t just growing, we were scaling with purpose!

In 2011, we expanded our global footprint, bringing systems thinking and teamwork strategies to healthcare leaders in Nigeria. It was a powerful reminder that safety is a universal language.

That same year, we launched StoryCareยฎ, a simulation-based storytelling tool developed with IDEAS to deepen emotional connection, trust, and communication in care teams. It wasnโ€™t just innovation. It was empathy in action.

Then came a defining moment. Synensys was selected to support the Air Force Medical Service (AFMS) Patient Safety Program, growing from 10 to 105 employees almost overnight. It was a leap in scale and in responsibility.

We also joined the Partnership for Patients initiative, supporting over 3,700 hospitals through the Hospital Engagement Networks. From 2011 to 2016, our work helped prevent more than 500,000 patient harms and save billions in healthcare costs.

2011 was the year we proved that scale and soul can grow together.

By year six, we werenโ€™t just training teams, we were transforming how they connect. With the launch of TeamSTEPPSยฎ Plusโ„ข, we reimagined teamwork through the lens of patient engagement, proving that trust, empathy, and communication arenโ€™t just soft skills- theyโ€™re safety strategies. Making patients and families part of the care team was a big Plus in reducing medical errors.

We didnโ€™t stop at training. We introduced coaching, guiding healthcare leaders to embed change, not just implement it. And when our work was featured in Health Affairs, the story of a newbornโ€™s life saved through simulation training became a national example of whatโ€™s possible when teams are truly prepared.

By year five, our mission was no longer just expandingโ€”it was evolving to meet the needs of diverse cultures, communities, and care settings.

In 2009, Healthcare Team Training customized a human factors and patient safety workshop for the North Carolina Quality Center, tailoring our approach to regional healthcare leaders and proving that one-size-fits-all doesnโ€™t apply to better safety in healthcare.

At Naval Hospital Jacksonville, we supported TeamSTEPPS implementation that directly impacted patient outcomes, including in maternity care, where teamwork was credited with saving lives.

And in a major leap toward global adaptation, we delivered our first TeamSTEPPS training in Japanese in Kamogawa City, a coastal town near Tokyo on Japanโ€™s Chiba Peninsula. This milestone wasnโ€™t just about translation, it was about cultural responsiveness, collaboration, and the belief that patient safety should be accessible to everyone, everywhere.

2009 reminded us that safety isnโ€™t just a systemโ€”itโ€™s a shared language.

By 2008, our mission had evolved from training teams to transforming entire systems.

As Healthcare Team Training, we introduced powerful tools that helped healthcare and defense organizations measure what mattersโ€”teamwork, safety culture, and performance. The launch of the Safety Attitudes Questionnaire (SAQ), TeamSTEPPS Teamwork Perceptions Questionnaire (T-TPQ), and Teamwork Observation Tool gave leaders the ability to turn frontline insights into system-wide improvements.

This wasnโ€™t just innovation, it was infrastructure for change.

We also deepened our thought leadership. Dr. Stephen Powell co-authored two major research publications, including โ€œCreating a Systems Approach to Patient Safety Through Better Teamworkโ€ and โ€œUnderstanding Quality and Safety Problems in the Ambulatory Environment.โ€ These works helped shape how safety was understood and measured in emergency, inpatient, and outpatient settings.

And in a key move toward systems integration, we began implementing AHRQโ€™s Hospital Survey on Patient Safety Culture (SOPS) giving organizations a clearer view of how their teams perceived safety, communication, and collaboration knowing culture is essential to safety management systems.

2008 marked a shift: from training individuals to transforming systems through data, insight, and action.
