US Air Force Medical Service: Synensys has partnered with the Air Force to provide Patient Safety Specialist programming for 10+ years resulting in a 70% reduction in preventable patient harm events. Many of the 70+ medical treatment facilities have reduced serious safety events to zero as a result of the full implementation of system safety practices and high reliability principles. US Air Force Air Mobility Command: Air Mobility Command has been able to develop a customized Line Operations Safety Audit (LOSA) program capable of interfacing and integrating with multiple safety management data sets leading to comprehensive system improvement in safety culture, safety reporting, and threat and error management performance. WellSpan Health: WellSpan developed a comprehensive system safety program for their largest hospital perioperative service line. Synensys Completes C-17 Line Operation Safety Audit (LOSA) Observer Workshop at Scott Air Force Base.

Synensys had the privilege of training 16 C-17 aircrew members at the C-17 Flight Deck & Loadmaster LOSA Observer Workshop from January 6–8 located at the Air Mobility Command (AMC) Flight Safety Headquarters, Scott Air Force Base, Illinois.

This workshop brought together Air Force National Guard, Air Force Reserve, and Active Duty members, including 10 C-17 Pilots and 6 C-17 Loadmasters, to prepare them for their roles collecting critical data for the Line Operations Safety Audit (LOSA) program, a highly successful initiative currently enhancing operational safety across AMC.

Over three days, our team:
✅ Delivered LOSA program training including Threat & Error Management fundamentals
✅ Demonstrated our LOSA software, SAFESIGHT, and guided hands-on exercises
✅ Conducted Capstone assessments and inter-rater reliability testing

With training complete, these observers are now deploying worldwide for 45 days of observations which will then be analyzed for actionable insights, supporting AMC’s global safety mission.

Synensys is proud to announce it has successfully achieved the Cybersecurity Maturity Model Certification (CMMC) Level 2 Final status, validated by an authorized Certified Third-Party Assessment Organization (C3PAO).

This certification signifies a major milestone in Synensys’ ongoing commitment to defending Controlled Unclassified Information (CUI) and safeguarding national security.

CMMC Level 2 certification demonstrates Synensys has fully implemented all 110 cybersecurity controls outlined in NIST SP 800‑171, verified by rigorous third-party assessment. This level of cybersecurity preparedness ensures high-order protection across 14 control families, including access control, incident response, audit and accountability, and data protection.

Throughout the past 20 years, Synensys continued to lead with purpose by completing landmark safety work, earning top marks for performance, and laying the groundwork for a secure, innovative future.

We completed and presented the first ever KC-46 Flight Deck and Boom Operator LOSA, supporting strategic air refueling missions with actionable insights to strengthen crew safety and situational awareness.

In April, Mark and Steve shared findings at the AMC Safety Conference from a LOSA study titled “How Situational Awareness Impacts Threat and Error Management.” That same month, we launched a new website, a fresh reflection of our evolving focus on tools, data, and system safety innovation.

In May, 73% of eligible military hospitals supported by Synensys received an “A” grade in the Leapfrog Hospital Safety Grades, up from 43% the previous spring. This improvement was a powerful testament to the impact of our work in patient safety and culture transformation.

We earned an exceptional CPAR for our LOSA team, extended our BUMED/Navy contract, and began supporting the DHA DAD-HA Patient Safety Program under a new contract awarded on August 1.

To close the year, Synensys achieved Cybersecurity Maturity Model Certification (CMMC) Level 2 Self-Certification. This certification reinforces our commitment to secure, reliable operations as we prepare for the future.

2025 was proof that when safety, performance, and innovation align, the future becomes something we’re ready to meet.

In year twenty, Synensys closed its second decade with impact, recognition, and a renewed commitment to advancing safety through innovation.

In July, we published a case report in JAMIA analyzing laboratory data transmission between two healthcare institutions using a widely adopted point-to-point health information exchange platform. This research reinforced our dedication to improving data reliability and interoperability in public health.

In September, we were awarded the OASIS+ SDVOSB contract, followed by the OASIS+ Total Small Business award in December. These awards secured our place as a trusted federal partner in system safety and high reliability consulting.

We also completed a Proactive Safety Analysis for the FDA SHIELD program, focusing on over the counter and point of care laboratory data quality. This work applied advanced safety engineering to one of the most critical areas of public health infrastructure.

And in December, our use of STPA (Systems-Theoretic Process Analysis) was officially referenced in the Sequoia Project’s Implementation Guide v2.0! It was a powerful validation of our influence on national standards for health data safety.

2024 was more than a milestone. It was proof that thoughtful, systems-based safety work can shape the future of healthcare, defense, and beyond.

In year nineteen, Synensys continued to break new ground: expanding our safety methodology, earning national recognition, and advancing federal research that could shape the future of public health.

We completed the first ever HMMWV LOSA, bringing our observational safety approach to tactical ground operations! We also signed a second multi-year LOSA contract with the U.S. Air Force, reaffirming our long-term commitment to aviation safety and operational excellence.

In March, our SAFESIGHT™ LOSA platform was featured in The Mobility Forum, spotlighting our leadership in data-driven safety innovation. The article explored how fatigue impacts threat and error management during LOSA observations, which is a powerful reminder of the human factors at play in every mission.

We also presented at the MIT STAMP Conference, sharing findings from our FDA research project on improving lab data exchanges using System Safety Engineering. In partnership with Dr. Nancy Leveson and Dr. John Thomas, we applied CAST, STPA, and IV&V analyses to uncover information loss and recommend safety-driven improvements.

And in September, we began supporting the VA Data Integrity and Agility Team as a subcontractor, applying advanced safety engineering to improve data reliability across the VA system.

2023 was proof that when innovation is rooted in purpose, it earns its place in the spotlight!

In year eighteen, Synensys expanded its safety mission into new domains, from strategic command centers to public health infrastructure and tactical ground operations.

We completed our first Air Operations Center (AOC) Line Operations Safety Audit (LOSA), helping the U.S. Air Force identify risks and improve performance in high-level command environments. This work brought our observational safety methodology to the heart of strategic decision-making.

In September, we launched a groundbreaking research partnership with the FDA’s Center for Devices and Radiological Health. Awarded under a Broad Agency Agreement, the project focused on improving the reliability, interoperability, agility, and quality of laboratory data exchanges using system safety engineering. In collaboration with MIT’s Dr. Nancy Leveson and Dr. John Thomas, we applied STPA (Systems-Theoretic Process Analysis) to one of the most critical areas of public health infrastructure.

And in November, we extended our LOSA methodology to tactical ground operations with the first ever HMMWV LOSA contract from Air Mobility Command! Reinforcing our commitment to safety wherever missions unfold.

In year seventeen, Synensys stepped into new territory, expanding the reach of safety innovation across air and ground operations.

On March 1, we completed the first-ever rotary-wing Line Operations Safety Audit (LOSA) for the UH-1N Huey, supporting the U.S. Air Force Global Strike Command and Air Force District of Washington. The Huey has served strategic missions for over 50 years, and this LOSA helped ensure its legacy continues with enhanced safety and performance.

We also launched SAFESIGHT™, our proprietary LOSA software platform designed to deliver real-time, data-driven safety insights. This marked a major leap forward in how we support aviation and healthcare teams — turning observations into action with speed and precision.

Later in the year, we completed our first “under wing” LOSA for C-17 Line Maintenance, expanding our observational safety methodology to include ground operations and maintenance teams.

2021 was a year of first, and a reminder that innovation begins when we listen, observe, and act with purpose.
