Capstone Hospice serves patients and families within a 50-mile the radius of Peachtree Corners, Ga. Our care is provided primarily in a patient’s private residence, thus meeting the desire of most Americans to die at home surrounded by family and loved ones. In addition to a private home, one’s residence may also be an assisted living facility, personal care home, or nursing home. In essence, Capstone Hospice wishes to recapture the spirit of the original hospice movement — local providers providing care to local members of the community in a home-based setting

We have competent, well-trained staff who are experienced and responsive. An expert is on duty around the clock to answer your questions, help to resolve problems or be on their way to make an in-home visit if necessary.

Meet our new Capstone Hospice volunteer, Kelli Clay.

During her very first visit with our patient and his wife at Celebration Village Forsyth, Kelli wore her self-decorated jean jacket, filled with color, creativity, and personality.
After some initial conversation, the patient’s wife smiled and shared her own self-embroidered jeans, proudly pointing out how perfectly they matched Kelli’s jacket. What started as a simple observation quickly became a moment of laughter, storytelling, and connection.

This is hospice — the Capstone way.

Along with the important care plans, doctor’s orders, or many visits, these small, unexpected moments remind our patients that they are seen, understood, and not alone.

These little connections our Capstone Hospice volunteers make, visit after visit, often grow into meaningful relationships with our patients, families, caregivers, and staff.

Celebrating Harold James – 2025 Volunteer of the Year

Harold was one of Capstone Hospice's first volunteers nearly nine years ago — and he’s still faithfully serving today.

He has seen hundreds of patients over the years, and our patients and staff absolutely love him. Harold has a steady presence and a genuine compassion that can’t be taught. He simply shows up and cares — and that is what Capstone is all about.

We recently honored him at a special luncheon and presented him with our 2025 Volunteer of the Year Award. It was a joy to celebrate someone who has given so much of himself to others.

This month, Capstone Hospice is honored to recognize Alethea Jacobus as our Hero of the Month. Alethea’s journey into hospice care is deeply personal and rooted in faith, compassion, and a profound sense of purpose.

Alethea describes her calling to hospice as something only God could have orchestrated. While she was a stay-at-home mom, she visited her best friend’s father in a South Carolina hospital, where he had been admitted to inpatient hospice. As she prayed for him before his defibrillator was deactivated, she experienced a moment that changed her life. She describes it as standing on holy ground—an overwhelming sense of God’s presence and love in that sacred space. That feeling has never left her. To this day, each patient visit carries the same reverence. For Alethea, every encounter is an opportunity to witness and reflect God’s love during one of life’s most tender transitions.

💙 A Mother-Daughter Heart for Capstone Hospice 💙

After connecting with Capstone Hospice, Laura began volunteering — and soon realized her daughter Charlotte’s gift for music could bless patients in a special way.

Volunteering has always been important in their family, so it felt natural for Charlotte to begin singing during visits. She loves sharing familiar hymns like “Be Thou My Vision” and “Come Thou Fount of Every Blessing.” The songs bring back memories for so many of our patients — and often for us, too. Sometimes patients sing along, nod with the music, or nurses pause in the hallway just to listen. Those simple moments create something truly special.

We’re so grateful and blessed for Laura and Charlotte and the comfort they bring through presence and song. 💙

Who doesn’t like a milkshake?

One of our big-hearted Capstone Hospice volunteers learned from a patient’s family member that the woman she was visiting at Brickmont of Roswell really loved Chick-fil-A strawberry milkshakes. On her next visit, the volunteer showed up with one — and it completely made the patient’s day.

A milkshake might seem simple, but moments like this are a good reminder of how much a thoughtful connection can mean to our patients, families, and Capstone Hospice volunteers.

At Capstone Hospice, it doesn’t have to be anything big. Often, it’s the small, intentional moments that matter most.

💙 Meet Joy — one of our favorite volunteers with four paws and a whole lot of heart. 🐾

Joy is a certified therapy dog who visits patients alongside one of our incredible volunteers, Paula Pinyan. She brings comfort in quiet ways — a gentle nuzzle, a calm presence, a moment of connection when words just aren’t needed.

During visits, Joy often helps ease anxiety, spark smiles, and create moments of peace for patients and families.

We’re so grateful for volunteers who share not only their time, but their hearts — and sometimes their dogs — with our patients. 💙

This month, Capstone Hospice proudly recognizes Palliative Nurse Practitioner Ashley Gaines as our Hero of the Month. Ashley’s thoughtful, compassionate approach to palliative care has made a powerful impact on patients and families navigating some of life’s most complex transitions.

Ashley chose to work in palliative care because she believes deeply in the importance of guidance and clarity during the end-of-life journey. Her focus is on helping patients build a bridge between curative and comfort care—offering insight, relief, and a smoother path forward. By supporting patients in making informed, thoughtful decisions, she plays a crucial role in improving their outcomes and overall experience.

At Capstone, Ashley has found a culture that aligns perfectly with her values. She describes the workplace as filled with support, joy, and mindfulness—qualities she says are rare in most medical settings. That sense of connection and shared purpose fuels her passion and strengthens the care she provides every day.

Outside of work, Ashley enjoys cheering on UGA football, practicing yoga, and being actively involved in her children’s lives by attending as many of their activities as she can. These moments of joy and movement keep her grounded and energized.

For her nonprofit of choice, Ashley has selected City of Refuge Atlanta, an organization committed to transforming lives and communities through support, empowerment, and faith-based service.

We’re honored to celebrate Ashley Gaines for her fine work and unwavering dedication. Her ability to bring both clarity and compassion to her patients is a true gift, and we are grateful to have her as part of the Capstone Hospice family.
