Effective June 27, 2025

Both our Peoria and Pekin offices will be permanently closing as of June 27, 2025. We are grateful for the trust you’ve placed in our team over the years.

Dr. Hanna – Relocating to a new area

Dr. Daccache – Illinois Vision Clinic, Peoria

Kimberly Lopez, APN – Carle Health Pekin Pain Clinic (Headache & Neck Pain patients)

Medical Records Notice:

*Important Announcement from Bond Eye Associates*

I am writing to inform you of an important and difficult decision regarding Bond Eye Associates. Due to unforeseen financial challenges, rising operational costs, and insurance reimbursement issues, we regret to inform you that our practice will be closing
permanently on June 27, 2025.

This decision was not made lightly, and we have explored every possible avenue to continue providing care. Unfortunately, the financial constraints have made it impossible to sustain the practice. We understand the impact this may have on your healthcare and are committed to ensuring a smooth transition. There is a Vision Care Practice that will continue to do comprehensive eye exams, contact lens exam, treat dry eye and other eye conditions and continue eye disease management. They will be seeing patients in our Pekin location only. Dr. Adler patients will need to arrange to have another physician take care of you. Headache and neck pain patients can find Kimberly Lopez, APN at Carle Health Pekin Pain Clinic.

To assist you during this time, we recommend that you contact your insurance company’s provider directory or check with the hospital’s physician referral service. Additionally, we will be available to help transfer your medical records to your new provider. Please call or text our office at 1-309-353-6660 or smccartney@bondeye.com for assistance with this process.
