Trinity Health PACE of Pensacola is part of Trinity Health PACE, one of the largest PACE providers in the country, with 25 PACE centers in 12 states, serving over 3,000 seniors. The day centers at each site are the heart of our care model with services including primary care, rehabilitative therapies, nutrition counseling, and more, all under one roof.
