💯 • Walk into a room while I am supporting a birth and you might think that I am just sitting there doing nothing. I won’t be talking. I won’t be touching the client. I won’t be using anything in my bag.

So what am I doing exactly?

🪑 Watching mom’s breathing and sometimes breathing loudly alongside her to remind her to slow down her breath

🪑 Scanning mom’s body during the contraction to make sure that she isn’t holding any tension

🪑 Monitoring the temperature and lighting to create the peaceful environment that she desires

🪑 Calculating how long it has been since she emptied her bladder

🪑 Thinking about how long she has been in her current position and thinking about the next position that she might try

🪑 Timing contractions to see if they are longer, stronger and closer together

🪑 Thinking about when mom and her partner last had a snack

🪑 Reviewing her preferences to make sure that everything is going according to her plan

🪑 Giving the couple space to connect on an intimate level (allowing for oxytocin) and/or guiding the partner to be the lead support

🪑 Updating notes and capturing labor flow

Sometimes just being fully present is enough. Sometimes just knowing I am there in close proximity is all mom needs to be able to relax and focus on what she needs to do. Sometimes just holding space for them in an unbiased manner allows for a noticeable shift in the energy of the room.

And the list goes on….

• You read that right.

Per the American College of OB/GYN's 2019 VBAC guidelines, the risk of maternal death during a planned vaginal birth after cesarean (VBAC) is 0.0019%.

Expressed another way, that is a risk of 1 in 52,600.

Did you have any idea the risk was that low?

Whereas the risk of maternal death with an elective repeat cesarean after one cesarean is 0.0096% or 1 in 10,000.

Again, both really small numbers but the risk of maternal death is five times higher in a repeat cesarean.

This is why when pregnant people are threatened with "you could die if you plan a VBAC," it falls flat.

But it is often really successful in coercing someone into a repeat cesarean... as well as eroding patient autonomy and any trust when the birthing person learns the facts.

As a L&D nurse told me, "There is no real informed consent anymore."

Many studies have challenged the idea of a fixed due date, with one researcher noting that this idea “stretches credulity.”

Gerald Wightman Lawson searched and analysed the medical literature relating to "variables on the length of pregnancy, the expected date of confinement, and prolonged pregnancy."

His research (like many other studies before) confirmed that:

"a number of factors were found to significantly influence the length of a pregnancy, including ethnicity, height, variations in the menstrual cycle, the timing of ovulation, parity and maternal weight." (Lawson 2020)

And for those who would like a bit more detail...

"The proposition that a pregnancy is 40 weeks or 280 days in duration is attributed to the German obstetrician Franz Naegele (1778–1851).

His rule adds nine months and seven days to the first day of the last menstrual period.

The expected date of confinement from this formula is approximately right in the majority of cases.

However, the idea that this rule can apply to every pregnant female – young or old, nulliparous or multigravida, Caucasian, Asian, African, or Indigenous – stretches credulity.

In addition, many women regard the 40‐week date as a deadline, which if crossed, may then place the baby under stress.

Forty weeks is such a simple, round, convenient figure that it has proved difficult to challenge, despite criticism.

Nonetheless, what might have been an appropriate formula in Germany in the 19th century deserves to be revisited in the 21st."
