We offer free prescription delivery, immunizations, blood pressure readings, bubble

packing of medications, private medication management consultations, compounded medications, pet prescriptions, GenoPath testing, and much more! POSITION REQUIREMENTS:

Evaluates physician medication orders for appropriateness of drug, dosage, potential interactions, route of administration and stability. Makes recommendations for therapy changes to the physician and facility staff as appropriate. Works with patient medical information to conduct Drug Utilization Review (DUR) on all prescriptions and review clinical edits to ensure there is no adverse drug interaction.

Maintains patient/resident database to include appropriate current and past medical history, appropriate clinical and laboratory data, and concurrent medication (including over-the-counter and home remedies). Works with appropriate pharmacy technician in reconciling Controlled Substances Log(s) with inventory on hand and prescriptions dispensed to comply with state and federal regulations.

Works with Chief RPh to ensure that implementation of cost containment strategies (Pre-Pack, Front-End Process, Cycle-Fill, etc.) occurs on all dispensing and procurement activities.

Provides clinical expertise by responding to drug-related questions and concerns initiated by internal (Technicians, Nursing Staff) and external (Directors of Nursing, Administrators) customers.

Collaborates with the healthcare team to provide clinical and technical support as needed.

Documents all communication with physicians, nurses, other members of the healthcare team and patients/residents and their family members in the medical record.

Obtains, as necessary, and maintains all required documentation in accordance with federal and state regulatory agencies and MD Group LLC policy.

Implements policies and procedures for the safe procurement, storage, distribution and use of drugs and biologicals, as well as disposal of drugs.

Assists facility/agency staff personnel with procedures relating to safe and effective methods of procuring, maintaining, administering and disposing of drugs.

We are blessed to be able to serve you during this challenging time. We want to keep you and our staff safe and healthy. If you are sick or think you have been exposed to the influenza virus or coronavirus, we kindly ask you to stay home. We WILL find a way to get you your medication!

There are many non-traditional ways to get your medicine that include FREE CURBSIDE PICK-UP and Mail delivery service if able for refills.

Call early... at least 3-5 days before you run out of your medicine to give us ample time to refill maintenance meds.

Try to keep distance when in crowds and try to avoid proximity with people who are sick.

Call and check-in with family, friends, and neighbors who may feel socially isolated and lonely. Our greatest gifts to one another are love and kindness.

Don’t panic. Use common sense. We will get through this as a community!

**This part-time position is for alternating Wednesdays and Saturdays in Peshtigo. There is also an opportunity for every other Friday in Kewaunee. Please apply even if you only are interested in one or the other location.**

Staff Pharmacist dispenses prescribed drugs in a retail environment. Also responsible for transferring prescriptions, handling incoming physician calls, logging data into the computer, and Drug Utilization Review. Requires a bachelor's degree and/or an advanced degree in pharmacy and is licensed to practice in Wisconsin. Familiar with a variety of the field's concepts, practices, and procedures. Relies on extensive experience and judgment to plan and accomplish goals. Performs a variety of tasks. Leads and directs the work of others. A wide degree of creativity and latitude is expected. Typically reports to a manager or head of a unit/department.

Preferred Education/ Experience

Bachelor’s degree from an accredited School of Pharmacy or Pharmacy Doctorate.

1-year experience in a retail pharmacy environment preferred.

Current/clear Wisconsin Pharmacist licensure.

Skills

Computer skills in MS Office Suite of products preferred.

Strong interpersonal communication skills including team leadership, problem and conflict resolution, training development and listening.

Action-oriented ability to identify and implement solutions.

Ability to simultaneously manage multiple, complex projects.

Peshtigo Hometown Pharmacy opened in September of 2015. Since that time, we have grown steadily with a reputation for strong customer service. We strive to do more than just fill prescriptions, focusing on the overall health of our patients. To meet all our community’s health related needs, we offer a variety of services including: free prescription delivery, immunizations, blood pressure screenings, nutritional supplements, medication therapy management and medication packaging for patients who need a little extra help managing their medications. We carry a full line of over the counter medications, vitamins, and herbal products. Our customers also enjoy browsing through our selection of seasonal gift items, home décor products, greeting cards, apparel and many other unique gifts. My name is Paul Farley. I grew up in Peshtigo and attended Catholic Central High School (now St. Thomas Aquinas Academy) in Marinette. I graduated from St. Norbert College with a degree in chemistry in 2003, then from UW-Madison School of Pharmacy with a PharmD in 2007. After spending a few years in Appleton, my family moved back to Peshtigo. In 2015, the opportunity arose to partner with Hometown Pharmacy to open our own pharmacy. Our focus is to provide outstanding customer service and emphasize the overall health of our patients, not to simply fill prescriptions. My wife Anne and I have have four children (all boys) along with 2 dogs. All of my free time is spent at various fields/courts watching our boys play football/basketball/baseball, arguing about fantasy football, and buying books that I don't have time to read.
