At Kindred Hearts, we aim to promote greater individual independence in the community one person at a time. This will be achieved through person-centered supports aimed to acquire, maintain, and improve self-help, domestic, socialization, and adaptive skills necessary to reside successfully in home and community-based settings while preserving health and safety. We have the knowledge, dedication and compassion to assist you in ensuring that you and/or your loved one’s needs are being met. Together we create an individualized plan that guides how your supports will be utilized. We are qualified through ODP to provide supports for individuals enrolled in the Person/Family Directed Support (P/FDS) and Consolidated waivers in Philadelphia, Delaware and Chester counties of Pennsylvania.
