ATTN PARENTS!!!
This is a friendly reminder that FRIDAY (tomorrow) will be Cloud Pediatrics LAST day. You will no longer be able to complete a records request in person after 5:00pm tomorrow, you will still be able to send a request for records via email to CLOUDPEDIRECORDS@GMAIL.COM until July 5th, 2024. If you have not completed a Records Released Authorization by then, you will be able to access your records through our records custodian. Be aware once records are transferred to the custodian there WILL be a FEE through them. We want to ensure that you have completed and returned your RECORDS REQUEST AUTHORIZATION as soon as possible to avoid this fee. Further information regarding our records custodian will be posted in the near future.
