Meet Dr. Phillip Nutter

Dr. Phillip Nutter, a native of Plum Borough, graduated from Plum High School in 1979. He began his journey in chiropractic care at the prestigious Palmer College of Chiropractic in Davenport, Iowa, in 1982, earning his Doctorate in Chiropractic in 1985.

Dr. Nutter has been proudly serving the Plum Borough / Penn Hills community in Pittsburgh since founding We Care Chiropractic Center in 1985.

Initial Visit
During your initial visit, you will receive a comprehensive consultation, examination, and x-rays. After the x-rays are completed, we will provide you with a detailed report of findings.

If your condition is suitable for chiropractic care, your treatment will begin the same day. However, if it is determined that chiropractic care is not appropriate, we will refer you to the necessary healthcare professional(s). These may include primary care physicians (PCPs), neurosurgeons, orthopedic surgeons, neurologists, rheumatologists, or oncologists.

Arthritis/Spinal Stenosis
What is Arthritis?

Saying someone has arthritis is like seeing a green food and simply calling it a vegetable—it’s a broad term that encompasses many types and degrees of joint conditions. The word "arthritis" is derived from the Greek roots "arth" (joint) and "itis" (inflammation). Technically, even swelling caused by stubbing your finger is a type of arthritis.

Understanding Osteoarthritis
The most common form of long-term arthritis is osteoarthritis. Joints in the body lack a direct blood supply, relying instead on motion and proper alignment to absorb nutrients, lubricate themselves, and eliminate waste products.

When a joint is misaligned or suffers trauma, this process breaks down, leading to:

Joint starvation (lack of nutrients).
Grinding or stickiness (lack of lubrication).
Over time, the body recognizes this dysfunction and compensates by altering the joint's structure. This can lead to the growth of bony spikes (osteophytes) and, in advanced cases, the formation of a bony cage that fuses the joint.

As chiropractors, we aim to intervene early by correcting misalignment and restoring proper motion. By reintroducing motion into the joint, the healing process begins. Nutrients and lubrication return, waste products are removed, and the joint’s natural life cycle is restored. This process can slow down, stop, and in some cases, even reverse osteoarthritis damage.

Other Types of Arthritis
Conditions like rheumatoid arthritis and gout are examples of other forms of arthritis. These may be caused by genetics, trauma, or lifestyle factors. While their underlying causes differ, many patients benefit from a combination of chiropractic adjustments and dietary changes to manage symptoms effectively.

Disc Problems or Disc Herniations
Understanding Spinal Discs and Disc Injuries

The human spine typically consists of 24 vertebrae and 23 discs situated between them. These discs contain a thick, jelly-like substance called the nucleus pulposus, which acts as a cushion, surrounded by strong annular fibers to keep it contained.

Unlike most joints in the body, discs lack a direct blood supply. Instead, they rely on motion—specifically, properly aligned motion—for nutrition and lubrication. Movement allows the joint surfaces to "work in" nutrients and lubrication while "working out" waste products. This is why staying hydrated (64 ounces of water daily) is crucial. Dehydration hinders joint lubrication, accelerating degeneration over time.

How Disc Injuries Occur
Disc injuries commonly result from either major trauma or minor repetitive trauma:

Major Trauma: Events like car accidents can tear annular fibers, causing the disc material to bulge, protrude, or herniate immediately.
Minor Trauma: Poor posture and repetitive actions can cause gradual degeneration. Over time, weakened fibers can fail, often leading to a sudden incident—like bending over to pick up a pen—resulting in a "pop" followed by back and leg pain.
An x-ray can sometimes indicate a disc injury, but MRI remains the gold standard for diagnosing its presence and extent.

Prevention and Treatment
Preventing disc degeneration requires a proactive approach, including:

Identifying and correcting weak or dysfunctional areas through chiropractic adjustments.
Strengthening the core to support spinal health.
Once a disc injury occurs, thorough evaluation becomes critical to determine the best treatment approach:

Conservative Care: Includes chiropractic adjustments, core strengthening, and axial traction (decompression therapy).
Aggressive Treatment: Surgery may be necessary if nerve damage is evident or worsening. Most orthopedic and neurosurgeons prefer trying conservative care first, provided there is no immediate risk of nerve damage.

While many people are familiar with traditional traction or inversion tables, modern computerized tables—such as the KDT table—offer precise control over patterns, timing, and intensity. This allows for maximum therapeutic benefits in a shorter time.

Decompression:
Metaphorical process: Refers to a reduction in intradiscal pressure (IDP) that may occur as a result of traction.
While decompression cannot be directly measured during each session, positive outcomes such as pain relief suggest its occurrence. "Decompression exists along a gradient. For example, lying down decompresses the spine compared to sitting."

Important Considerations
While some manufacturers claim their devices deliver negative intradiscal pressure, no current device can measure pressure in real-time during therapy.
Scientific Research: Organizations such as the FDA, Medicare, and Blue Cross acknowledge the difference between traction (definitive physical process) and decompression (assumed based on outcomes).
Positive Outcomes: Even if full decompression is not achieved, therapy often induces healing and symptom relief, making it a valuable tool in chiropractic care.
