ng unnecessary hospitalizations, reduce facility length of stays and decrease costly diagnostic tests and treatments that will not positively impact patient care. We work with our partners to ensure proper and safe transition for our patients. We also emphasize palliative services and are always keeping patient goals and values at the forefront of our treatment plans.

Our Story

Personal Care Medical Associates is a physician-led post-acute care provider network dedicated to improving the quality of life for patients in skilled nursing facilities, long term care homes, assisted and independent living facilities, personal care homes and hospitals. Advanced Pain Medicine for chronic care management and advance care planning extend PCMAs network. PCMA encompasses geriatric medicine, palliative and end of life care, physical medicine and rehabilitation, as well as interventional pain, with an out-patient clinic soon to be established in Western Pennsylvania.

Our goal is to care for the patient, in the patient’s environment. High quality, personalized care provided by our network of physicians can reduce unexpected healthcare costs such as unnecessary testing and re hospitalizations.
