We are currently seeking a Client Service and Intake Coordinator to manage client intake, oversee service plans, and coordinate schedules to ensure our clients receive the best possible care.
Position Overview:
The Client Service and Intake Coordinator will conduct consumer supervisory visits, create and manage service plans, assist clients with accessing necessary resources, and work closely with the Staffing Coordinator to develop and manage schedules. This role also requires the coordinator to be on call 10 days a month.
Key Responsibilities:
Conduct supervisory visits to clients' homes to ensure quality care and client satisfaction.
Develop and maintain individualized service plans tailored to clients' needs.
Assist clients in accessing resources and services that support their well-being.
Collaborate with the Staffing Coordinator to create and manage staff schedules.
Be on call 10 days per month to address any urgent client or staffing needs.
Maintain accurate documentation of all client interactions and service plans.
Manage the intake process for new clients, ensuring a smooth and efficient onboarding experience.
Qualifications:
Strong organizational and time management skills.
Excellent communication skills, both verbal and written.
Ability to work independently and as part of a team.
Proficiency in using computer software for scheduling and documentation.
A client-focused and empathetic approach to service.
