Coming home after inpatient rehab can feel overwhelming, but continuing your recovery at home is a big part of your progress. Start by setting small, achievable goals. These could include activities such as walking a certain distance each day, completing your home exercise routine, or becoming more independent with daily tasks like dressing or cooking.

To track your progress, try:
✔️Writing in a notebook or rehab journal
✔️Using a checklist or calendar to log your activities
✔️Taking photos or short videos to visually track improvements

Make time to reflect on what’s getting easier and where you're still facing challenges. Noticing small wins like less fatigue, improved balance, or finishing a task without help can boost your motivation.

If you received home exercise instructions from your therapy team, stick to them and update your notes as you go. Don’t hesitate to reach out to your doctor or care coordinator if you have questions or concerns.

Here are just a few highlights from across our locations:

• Christmas in July in Peoria brought together local healthcare professionals for an evening of conversation, collaboration, and connection.

• We were honored to celebrate a valued partner at the Touching Hearts 10-Year Anniversary event.

• Congratulations to Kally Mitchell, DPT, promoted to Director of Business Development, and Laken Ebarb, COTA, promoted to Senior Clinical Liaison at Reunion Plano.
