This patient had Mohs surgery for a basal cell carcinoma on his nasal tip. 👃🏻 Mohs surgery is the gold standard for treatment of skin cancers, especially in high real estate areas like the nose— it maximizes tissue preservation and has the lowest possible recurrence rate.

Every defect (aka "wound") that results from the removal of skin cancer requires a customized reconstructive solution. Here, we designed a nasal crescenteric advancement flap along with a Burow’s graft.
Essentially, the skin on the cheek was advanced (aka "pushed") into the wound to cover as much as possible. A skin graft was used to cover the remainder of the wound. Using adjacent skin provides the best match to the patient’s skin thickness, texture, and color. Not only does it need to look good, it needs to function: he has to be able to breathe out of his nose after all this is done!

One of my favorite dermatology professors used to always say “melanoma will make a fool out of you.” Melanomas can be sneaky and notoriously hard to spot, and can mimic a lot of other skin lesions. In this patient’s case, it looked a lot like a vague sunspot or solar lentigo.
To help identify melanoma, we sometimes use a “Woods lamp,” which uses UV light to make melanin in the skin more visible — swipe to see this same spot under the lamp. It’s much more defined!
Melanomas often make excess amounts of pigment - or melanin - which is an excellent absorber of UV light. This is what makes melanomas “light up” under UV light. It’s also why our bodies make melanin — to absorb and protect our skin from harmful UV light rays!
This is why (generally) people with less pigment or melanin in their skin tend to have more skin cancers (and wrinkles too). They just don’t have as much natural sunblock ☀️ @ The Surgery Center at Plano Dermatology

This patient came in for Mohs surgery to treat a basal cell carcinoma. Swipe to see pictures mid-procedure 👍🏻
A quick rundown on Mohs:
First, the visible skin cancer is removed and examined under the microscope by the Mohs surgeon. If there’s still cancer anywhere at the specimen’s edges, the surgeon goes back to take more until the cancer is cleared. This not only ensures the highest cure rate for the cancer, but also preserves as much healthy skin as possible.
Once the cancer is cleared, we turn our attention to the wound. Depending on its size, shape, and location, we employ a variety of techniques to minimize scarring; this patient was very pleased with his outcome!
While getting rid of cancer is incredibly rewarding, it’s also particularly satisfying to apply some creative reconstructive techniques and help someone look great again.

This ear looks pretty normal right? Swipe for the before 👉🏻— a huge change from the keloid my patient lived with for 20 years!
A keloid forms when scar tissue (aka collagen!) grows unchecked, usually after an initial injury to the skin. Keloids aren’t cancerous or contagious, but are often accompanied by significant itchiness, pain, and as you can imagine, social side effects.
Fun fact: While a keloid is not a cancer, it certainly can act like one. In fact, Cancer is Greek for “crab”, and keloid is Greek for “crab claw”. Both must be treated properly to stop growth and prevent return.
Our patient lived with 20 years of keloid growth, likely resulting from something as simple as an ear piercing. In 20 minutes, and under only local anesthesia at our clinic, this man’s keloid was surgically removed and his ear was back to normal. But our patient’s favorite part? “My grandkids won’t be scared to play with me anymore,” he said with a big grin. That’s a win in our book! 👂🏻 Have questions about keloids? Melanoma Monday: This is a melanoma of the nail unit in a Latino patient.

Melanomas on the hands and feet - aka acral lentiginous melanomas - have a worse prognosis relative to other types of skin cancer. This melanoma can affect any ethnicity.

While surgery and immunotherapy have come a really long way in treating melanoma successfully, early intervention and good routine screenings are the best tools in our battle against melanoma.
