“Despite the natural design of the body, labor doesn’t always progress as planned. You may hear terms like ‘failure to progress,’ which can feel disheartening. However, it’s important to reframe this language. Birth is rarely about failure—it’s often about misalignment, imbalance, or the need for additional support.
For example, issues such as a baby’s head positioned at an angle (asynclitic head position) or a hand placed near the head (nuchal hand) can increase discomfort and prolong labor. These positional challenges may cause:
Prolonged pushing: Pushing for more than one to two hours is often linked to malposition.
Shoulder dystocia: When the baby’s shoulder becomes stuck, requiring additional assistance.
Deep transverse arrest: A situation where the baby doesn’t rotate properly in the midpelvis, often resulting in a cesarean.
