Shuff was always the ‘Boss’ to me. From the first day we met, when I visited his OR in 2008, I walked away impressed by the work of a true master—elegant, simple, straightforward, with no wasted steps, and highly efficient.

He was, also, the coolest guy I knew—a rock star, a captain of industry. That was his own quip when asked how he got his Concierge Key card from American Airlines. Between traveling the world to educate and teach about spine surgery, rock concerts, fishing, and caring for tens of thousands of patients, he still made time to be my friend and mentor. There are very few people I’ve met in my life whom I respect more.

We were partners for ten years from 2010–2018, and again from 2020–2022. I learned a great deal from him. At times, we were oil and water—our personalities couldn’t have been more different—but my respect for him never wavered. His dry humor and quick quips still find their way into my own daily language.

Rest in peace, Boss. It was an honor to know you, to work alongside you, and to call you my friend.

For my colleagues,

Every week, I get a message or a call from a physician—usually early in their career—navigating a tough situation. Contract disputes, practice transitions, burnout, feeling lost in the system.

It’s not always about needing an answer. Sometimes it’s just about hearing that they’re not crazy for feeling stuck. That someone else has been through it.

The messages in this post aren’t unique. They’re a reflection of something we don’t talk about enough: how isolating this field can be—even for smart, capable, well-trained people.

To the more seasoned among us: take the call. Answer the message. Tell your story. Even the uncomfortable parts. Especially the uncomfortable parts.

It’s not only about posting online. It’s about showing up—for the next generation of physicians who are trying to find their footing in a system that wasn’t built for them either.

Most people don’t realize this, but most general AI platforms don’t remember anything unless you give them structure.

That’s why I use Project folders like mini custom apps.

Each folder is its own little workflow:
• One for op note generation
• One for content creation
• One for my “AI for Beginners” series
• One for summarizing journal articles
• One for billing + denials
(Yes, even that has its own AI workspace.)

When I stay inside that project, the AI remembers my tone, formatting, workflow, and preferences — because I’ve trained it there.

You don’t need to know how to code.
You just need to know how to stay organized.

If you want to get more value out of AI — give it memory and direction.

Project folders = memories
Project folders = workflows
Project folders = efficiency

Yesterday, I was talking on another platform about how I use AI in my day-to-day as a spine surgeon… and it sparked a lot of interest.

So I think it’s time to start sharing what I’ve been building.

Before diving into complex use cases, let’s start with the basics.

Especially for the old-school folks who are still a little skeptical — this isn’t about replacing you.
It’s about making your day easier.

Too often, frustration with AI comes from vague prompts, missing context, or misunderstood goals — not the model itself.

So what’s a prompt?
It’s simply the instruction or input you give the AI to get what you need.

Here’s a short, high-yield visual guide to prompting well:

Start small. Be specific. Review everything.

Getting the basics right helps you avoid hallucinations, bad outputs, and wasted time — whether you’re generating clinic notes, summarizing a journal article, or brainstorming a project.

It’s like surgery: your prep and setup dictate the flow of the case.
Same goes for your prompts.

This is part of my new AI for Beginners series — for physicians and surgeons who want to use AI effectively (and not get left behind).

And yes — all of this was created with the help of AI.
The slides, the copy, the visuals — everything. I just gave it direction.

Joining the Paley Orthopedic & Spine Institute

I am proud to announce that I have joined the Paley Orthopedic & Spine Institute in Ft. Lauderdale. This unique opportunity allows me to remain in the forefront of clinical research, technology development but most importantly provide a patient-centric concierge approach for complex spine care.
