Summit at Plantsville Center for Health & Rehabilitation We employ trained specialists equipped with a diverse set of skills who are ready to field the most unique challenges in your rehabilitation process.

About you
Our center provides guests with a variety of individualized services and programming, whether they are with us for a short stay following a hospitalization or surgery, or they will call us their new home for long-term nursing care. Our dedicated staff is focused on delivering the highest quality of services, with a common goal of helping guests become better, brighter and stronger versions of themselves.

• Passport TM Approach •
Our Passport TM Approach is a natural progression of our services, helping our short-term rehabilitation patients return to the best quality of life possible, as quickly as possible. The approach we take optimizes recovery through individualized occupational, physical and speech language therapy plans with goals tailored to their specific needs and preferences. The ultimate goal of our approach is to restore you to health. We understand that every person under our care has distinct requirements for therapy. Your individual rehabilitation is tailored to a series of realistic, short-term achievements based on getting you home. We encourage you to meet with our interdisciplinary team of specialists to discuss your goals and expectations prior to your stay. With a patient-centered approach to care, our program is designed to provide you with the skills and abilities you need to live a healthy and fulfilling life.

• Long-Term Skilled Nursing Care •
All National Health Care Associates affiliated centers also specialize in delivering quality long-term care to residents. The team develops a comprehensive care plan which includes all medical, nursing, rehabilitation, nutritional, psychosocial, and therapeutic needs. Each resident is also evaluated upon admission by our interdisciplinary team, based on need for rehabilitative services in physical, occupational and/or speech therapy. Thereafter, a team of therapists carry out restorative or maintenance therapies in conjunction with the resident’s plan of care. Our goal is to assist each resident with the means to maintain his/her optimum level of functioning.

• Alzheimer/Dementia Special Care •
This program is designed to meet the special needs of Alzheimer’s/Dementia residents in a safe, caring and secure environment. Our goal is to help residents with Alzheimer’s or related dementias maintain their current level of functioning.

We’re collecting non-perishable food items now through December 17 to support our local food pantry.

Donations can be dropped off anytime during the week at 261 Summit Street, Plantsville, CT. Suggested items include canned vegetables and fruit, soups and stews, pasta, rice, cereal, oatmeal, crackers, shelf-stable milk, pasta sauce, and more.

The Annual Holiday Christmas Shop is returning to Summit at Plantsville! 🎁

We’re now accepting new and like-new donations through December 8 to help stock our resident holiday store giving residents the joy of selecting gifts for their loved ones at no cost.

Our Wall of Honor ceremony yesterday was a moving tribute to the brave men and women who served our country.

From the Presentation of Colors and the National Anthem to the thoughtful words shared by our guest speakers, every part of the program reflected deep respect for our veterans. Certificates were presented, stories were shared, and we took a moment of silence followed by TAPS in their honor.
