The Foundation was established in 1990 to provide an important additional facet to healthcare - Community Outreach. The Foundation annually funds programs, which provide and support community health education and wellness as well as patient care enhancement initiatives. The Tree of Life at the CVPH Kidney Dialysis Unit stands as a beautiful symbol of strength, resilience, and hope. It recognizes both the patients who face challenges with courage and the dedicated staff who provide compassionate care every day.

You can add a leaf to the Tree of Life to honor a kidney dialysis patient or a member of the care team who has made a difference.

Jane’s Fight Fund: Support When It’s Needed Most 💚

Jane’s Fight Fund was created in 2019 by the Sample family in honor of Jane Sample, who was diagnosed with Stage 4 stomach cancer in November 2016. Jane’s strength and spirit continue to live on through a fund dedicated to helping others facing similar challenges.

The fund helps ease financial burdens and lift the spirits of families by offering giveaways three times each year. Recipients may use the gift in whatever way is most helpful to them.

Who is eligible:
• Currently undergoing cancer treatment
• Resident of Clinton, Franklin, or Essex County, NY
• Must complete an application for each drawing
• Each applicant is assigned a number to ensure confidentiality

We had the privilege of recognizing two incredible caregivers through The Foundation of CVPH’s Honor a Caregiver Program.

Grateful patient Nancy honored Jennifer MacLean and Alyssa Pytlak as Health Care Heroes, sharing her appreciation for the care they provided. She described them as motivating, pleasant, and truly supportive throughout her experience.

The Honor a Caregiver Program gives patients and families the opportunity to recognize team members who go above and beyond, and moments like this remind us how powerful compassionate care can be.
