Today’s the last day of the AALNC forum and what a great experience it has been!

It has been amazing to meet the LNC community in person…

I’ve learned a ton, made lots of great connections, and am more passionate now than ever about my legal nurse business. Now I get to stay for the life care planning conference!

Psychiatric malpractice cases can get messy but it’s not because they’re weak.
They’re messy because the *records* are.

If you represent plaintiffs in psychiatric cases, here are the 3 biggest documentation challenges I face:

1️⃣ Fragmented timelines
Care is spread across inpatient units, outpatient therapy, ED visits, PCPs, telehealth, and more. No one record tells the full story — which makes patterns of deterioration easy to miss.

2️⃣ Checkbox su***de risk assessments
“Denies SI.”
“Contracts for safety.”
Template risk levels without clinical reasoning.
When foreseeability matters most, documentation is often vague or conclusory.

3️⃣ Medication management gaps
Failure to monitor side effects.
Delayed med changes despite decompensation.
No clear connection between prescribing decisions and symptom escalation.

The defense says: “The patient was stable per documentation.”
But behavior logs, nursing notes, and the medication timeline often tell a different story.

My role is to reconstruct the psychiatric timeline, identify where the standard of care breaks down, and translate subjective psych records into a clear liability narrative.

Because in psychiatric litigation, clarity wins cases.

Sometimes you find yourself with a choice to make:

Do I pick the easier route because I’m comfortable and don’t want to disrupt my routine?

or

Do I do the thing that’s scares the *bleep* out me and potentially evolve as a human being?

There’s no right choice - everyone is different.

But personally, I’m team 𝘥𝘰 𝘪𝘵 𝘴𝘤𝘢𝘳𝘦𝘥. If I let fear make my choices for me I'd never be where I am today 🙂‍↕️ 🙂‍↕️
