Low back pain CPGs - are you up to date?

Highlights:
🏋🏼‍♀️Exercise is king! Every client should be exercising. If they’re not, you’re doing them a disservice. Not much of a consensus on type or dose (benefits from both strength if and aerobic), but the biggest thing is to just get them moving.

✋Manual therapy can and should be used to reduce pain and temporarily improve mobility to support active exercise. People who hurt less (even in the short term) move more and move better. Can include spinal mobs, soft tissue mobilization, and dry needling.

❌Do NOT use mechanical traction - very low level evidence of ambiguous efficacy.

🔥Hot new research from BJSM 🔥

How often do healthcare providers, or even patients themselves, delay starting physical therapy for arthritic joints because “the arthritis isn’t going anywhere” or because “it’s symptom management, not curative” all with the mindset that this journey only ends in a joint replacement? All the time! But, they may be doing more harm than good.

This February 2023 study highlights how delaying physical therapy, even by as little as a month, significantly increases a person’s risk of opioid use.

Not only can physical therapy reduce pain and improve function in those with arthritic joints, it can also delay joint replacement and make recovery from joint replacement easier!

More movement = less pain!

The world of chronic pain treatment is inundated with drugs, injections, passive modalities, and an overarching message of “do less.” While there is a time and a place for these treatment methods, study after study has proven that the “do less” message has little value in modern chronic pain management.

In fact, the message needs to be “do more!” More daily activity. More recreational activity. More structured exercise. Movement is not something to be feared or avoided, but instead encouraged as a safe and effective means of managing pain and optimizing function!

We’ve talked about patient preference before - the often left out, third wheel of the clinical decision making triad. While evidence based medicine gets the glory, patient preference is often the make or break factor when it comes to successful clinical practice!

Patients care about what is happening to their body. More and more, they’re speaking up, asking questions, advocating for themselves, making their desires known. For those with chronic pain, effectiveness of the treatment and ability to improve performance of daily activities, safety risks, and costs heavily impact patient preferences. Treatment frequency and how quickly a treatment works were of secondary importance.

Take these factors into account when interviewing your new patient, especially if they’re on the fence about physical therapy. In the hands of a skilled provider and motivated patient, physical therapy can hit every one of these nails on the head - safe, effective, fast, and lower healthcare costs.

If you could make your surgical recovery easier, wouldn’t you? Pre-surgical rehabilitation enhances post-surgical strength and reduces length of hospital stay in patients with a total knee replacement.

Let’s talk hybrid DPT programs. They’re touted as being less expensive and more accessible than traditional programs. But are they on par with the academic rigor of a traditional program? Do they turn out capable entry level PTs, especially when it comes to hands on skills? And, perhaps most importantly, what does this say about the direction our profession is heading?

For the first two questions, it remains to be seen (but I doubt it). Hybrid programs are new, and many aren’t accredited yet. Those that are accredited cleverly advertise that they’re ranked such and such a number in the country…except that’s for their in person program, not the hybrid.

After years of fighting for more education, autonomous practice, more respect from the general public and our peers in healthcare, how can we intentionally cheapen and devalue our profession? Send ourselves back to the days of being a technician? Condensing critical hands on skills to a few days or weeks of in person learning? Because that’s what this is. Universities attempting to minimize the cost of a service, while being able to enroll more students, and not giving two 💩 about quality of provider they will turn out. Follow the money.

They screwed our profession once. Let’s not allow them to do it again.

Physical therapy is key to battling the opioid epidemic!

Many patients fear becoming addicted to opioids, especially following a major surgery. Physical therapy has been proven time and time again, across many body regions and surgery types, to reduce the need for post-op narcotic medication. Even better, the “side effects” associated with physical therapy include improved mobility, general function, and strength!
