Empty nose syndrome is often misunderstood.
Patients may feel like their nose is wide open yet still struggle to breathe. Symptoms can include dryness, air hunger, anxiety, and discomfort.
A recent systematic review supports a structured, stepwise approach — starting with conservative management and carefully selecting surgical options when appropriate.
Thoughtful evaluation matters.

If you stopped LPR treatment after one month because it “didn’t work,” this research matters.
A prospective study showed only about one-third of patients improve in the first month — but cumulative success at nine months reached 82–99%.
LPR management is often effective. It just takes longer than most people expect.

If your silent reflux symptoms are worse at night, there’s usually a pattern behind it.
Late meals, lying flat, and evening habits all increase exposure when throat tissue is most vulnerable. Nighttime recovery depends on timing, positioning, and consistency.
Small structural changes in the evening can make a measurable difference.

If you suspect silent reflux, start with the Reflux Symptom Index (RSI).
It’s a short, validated questionnaire that helps quantify throat-related reflux symptoms — and research shows RSI scores correlate reasonably well with 24-hour pH probe testing.
Structure beats guessing.

The most common question I get about silent reflux is simple:
“What should I actually do each day?”
Not theory. Not long food lists. Just a practical daily structure.
LPR symptoms are often driven by cumulative exposure — meal timing, portion size, evening habits, sleep positioning, and throat behavior all stack throughout the day.
So I organized these research-supported principles into a simple morning-to-bedtime framework.
The LPR Focus Checklist is now available.

Over the last few weeks, many of you have asked the same question:
“What actually works for silent reflux?”
Research consistently supports structured lifestyle modification — not random diet changes. Meal timing, portion size, sleep positioning, stress regulation, and throat behavior all play a role in reducing cumulative irritation.
So I organized the evidence-based principles into a simple daily checklist format to make it easier to follow consistently.
The LPR Focus Checklist is now available.

Chronic cough lasting more than 8 weeks can be incredibly frustrating.
A recent state-of-the-art review highlights that many cases of refractory chronic cough involve cough reflex hypersensitivity — where even small triggers set off persistent coughing.
Understanding the mechanism changes the approach from chasing infection to calming the reflex.

Many patients say their sense of smell feels too strong — overwhelming, intense, even disturbing.
Interestingly, research shows that many of these patients actually test as less sensitive on objective smell testing. This suggests altered or distorted smell perception, not true hyperosmia.
That’s why objective testing is so important when evaluating smell complaints.

A normal scope doesn’t always mean your throat symptoms aren’t real.
LPR symptoms don’t always match what we see on exam.
Reflux can cause irritation without dramatic visible changes.
Good care looks at the full picture — not just one test.

Throat clearing. Chronic cough. Hoarseness.
LPR can be incredibly frustrating — especially when tests don’t give clear answers.

New research shows there’s no single symptom, score, or exam finding that diagnoses LPR on its own.
That’s why good care looks at patterns, not just one test.

If your symptoms are affecting your quality of life, keep asking questions.

😮‍💨 Chronic sinusitis affects millions — but most people aren’t sure when to consider sinus surgery.
The new AAO-HNS guidelines give a clear answer:
When symptoms continue despite good medical therapy and imaging confirms inflammation, surgery can finally open the pathways so medications work again.
You don’t have to live congested forever.

Checking in from Curaçao 🇨🇼 at a pediatric ENT session today — and the big takeaway was that kids get dizzy for totally different reasons than adults.

They rarely get BPPV or Ménière’s.
The big ones are vestibular migraine, inner ear malformations, vision issues… and something called Recurrent Vertigo of Childhood, which makes up about 20% of cases and thankfully kids usually outgrow.

Treatment looks a lot like migraine care: good sleep, hydration, steady meals, low stress, and sometimes supplements like MigreLief (magnesium, riboflavin, feverfew).

Hope this helps parents understand what’s going on behind the scenes. More Curaçao updates coming.”
