Community-Based Rehabilitation, or CBRS, is a person-centered approach to help those living with mental health conditions live meaningful, independent lives within their communities. It emphasizes empowerment, inclusion, and support, using local resources and collaboration rather than institutional care. Our CBRS providers at Portneuf Valley Family Center use this approach to help our clients reduce stigma and social isolation, foster independence and skill development, and strengthen community participation and engagement.

Meet Kurt Blaisdell, CBRS and Case Manager at PVFC! Kurt is a Certified Recovery-Based Specialist with 4+ years of experience in the mental health field. He specializes in advocacy, skill building, community integration, and personalized recovery planning. Originally from Southeast Idaho, Kurt uses professional expertise and local insight to connect our clients with the right resources to aid in their recovery. Outside of work, you can find him camping, golfing, hunting, or exploring Idaho in his UTV. Thank you for all that you do, Kurt!

Meet Trinety! She graduated in May 2024 with her master’s in social work and brings four years of experience in the foster care system, direct care, CBRS, and case management. Now at PVFC, Trinety provides therapy for children, teens, adults, couples, and families. She’s currently training in trauma-focused CBT, trauma therapy, and Gottman Method couples counseling, although she already incorporates a wide range of therapeutic approaches. Trinety is passionate about supporting the neurodivergent community, helping clients gain understanding and build new skills in a safe, supportive space where they feel heard and accepted. When she’s not in the office, you’ll find her soaking up the sun, playing games, hiking with her pets, or spending time with family. Thank you for all that you do, Trinety!
