RN -
As a Case Manager or a Field RN (doing visits) you will make a difference in the lives of our patients. Complete an initial health assessment of patient and family to determine home care needs. Conduct home visits as assigned, assessing the patient’s condition, progress toward goals, providing patient care, teaching and other skilled interventions as ordered. Administer medications and treatments as prescribed by the physician.

Aide -
Supply personal companionship, remaining actively engaged with each client by providing ongoing social & emotional support. Complete daily tasks in a safe and professional manner (ADLs).
Assist with personal care activities to include personal hygiene, mobility assistance, incontinence care, light housekeeping, feeding and meal preparation.

Home Health Nurses provide a variety of tasks during patient visits depending on the specific plan of care and type of patient. They often have the following duties and responsibilities:

-Assess and chart observations of the patient’s condition at each visit.
-Complete evaluation tasks, including reviewing medication and vital signs.
Administer Physician-prescribed medication.
-Dress or redress wounds and assess the healing progress.
-Provide education to patients and families on proper home health care procedures and strategies.
-Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient’s care plan.
-Keep the patient comfortable through palliative care.
-Recommend tools or devices that may improve the quality of life for the patient.

You may be assisting the Clients with daily living activities such as dressing, bathing and meal preparation. There may be light housekeeping involved. Also, driving Clients to Doctor appointments or errands.

Home Health Aides help patients in a variety of ways, which may include taking their vital signs or giving them medicine under a nurse’s direction. They must record and report this data as medically necessary. Aides also help patients with their daily functions, such as dressing or taking a bath. They can assist with planning for doctor’s appointments or other meetings and may organize transportation for them.

Other Home Health Aide responsibilities may include:

Housekeeping, such as vacuuming or doing laundry
Cooking meals
Grocery shopping
Reporting to nurses about patient’s health status
Teaching patient self-care skills
