Please be aware that we are currently having issues with our fax machine. We are not receiving prescription refill request from the pharmacies by fax. Please call your prescription request directly to the office by dialing 337-585-7898. Select option 1 for Dr. Tate and option 2 for Dr. Fandal. We are sorry for the inconvenience!!!
