We wish to tell our patients that Dr Azima is retiring after 45 years in practice effective September 3, 2021. The office will be officially closed after that date.
Records may be obtained by calling 941-629-3636 to arrange for pickup or mailing or email nurse@swflwomensclinic.com to have records emailed to you or a provider.
Dr Azima may see some long time patients on a limited basis for limited services. However no new patients will be seen and no terminations will be provided.
We wish him an enjoyable retirement.

We are sorry to tell you that we have lost our lease in our North Fort Myers location due to the protestors. To date we have not been able to find a new location in Fort Myers. So at the present time we are seeing patients at out Port Charlotte location only.
