BIO:

Dr. Sokunthea (Sukie) Nau is a lifelong runner with a special interest in the biomechanics of athletic performance. He earned his Doctor of Physical Therapy degree from the University of Saint Augustine and uses his knowledge of the musculoskeletal system to increase the optimal functioning of the human body. His philosophy is a holistic approach to treating the underlying cause of movement dysfunctions. Sukie holds a high commitment to serving the community's Physical Health issues. In addition to treating patients, he is also the president of Miami Runners Club, a club that welcomes athletes of all ages and ability levels, and prides itself of the diversity of it’s members. In a multi-cultural community, they are a force that brings it’s people together with the common goal of spreading the word of fitness and good health throughout South Florida and beyond. Pregnant and Uncomfortable? Physical Therapy May Be Your Answer!

Pregnancy creates changes in your body, and those changes can make a woman uncomfortable.
Your belly gets bigger, and as that happens, your center of mass changes, and your body balance changes. That creates issues which may cause discomfort in a pregnant woman. Also, there are hormonal issues such as a relaxer that is released toward the end of pregnancy which may cause parts of the body to be relaxed that shouldn’t be such as the sacroiliac.
Physical therapy for a pregnant woman is working on the soft tissue, relieving the knots in the muscles that get even more tight than usual, and distribute the pressure. We work on the trigger points to realign the pelvis, and decrease the stress of the body so that the mother-to-be will be able to bend and twist and sleep with less pain.
Some women have “Diastasis recti” which means your belly sticks out because the space between your left and right belly muscles has widened.. You might call it a pooch. It's very common among pregnant women. About two-thirds of pregnant women have it. Physical therapy can bring the separation closer together.”
Other issues may include incontinence and pelvic pain. Physical therapy can release tight pelvic muscles, and help prevent prolapse of the bladder, and other organs from dropping down. We also teach mothers exercises to
keep their pelvic floor strong.
If a mom is healthy after the birth of the baby, we continue to see her..
We even treat her newborn baby if he/she has wry neck which means his/her neck gets stuck in that position it had in the womb. Physical therapy can do that right after birth for the baby. We can help clear the entire issue, improve the range of motion, and create better balance and better symmetry.
For a healthy, happy pregnancy right from the start, physical therapy can begin as early as the first trimester when the musculoskeletal is stiff, and continue through each stage of pregnancy right up to the end. Physical therapy is a preventative action to make sure that a mom-to-be is as pain-free as possible.

The most effective work we do is with trigger points after pain gets misdiagnosed.
In most cases of pain, trigger points are not properly addressed.

Trigger points are hyperactive muscle spasms caused by strain, muscle overuse where the muscle protects and guards itself.

The methods I use include:

-Overlap of trigger points at acupressure points (Chinese);
-Marma points in yoga overlap all of those points which are included in ayurvedic
medicine as in India;
-Universal points help people get better as well.

Targeting trigger points help:

-Vertigo;
-Headaches;
-Sciatica;
-Frozen shoulder.

These trigger points can be illusive:

Satellite trigger point starts at the shoulder runs down the arm into hand. The central point needs to be addressed.

Post therapy, surgery, injections and constant pain there are certain trigger points you can count on to be there.

All people have some tendencies to have the same trigger points. When the trigger points are targeted, there’s a 80-90% of pain reduction.

You can even learn how to self-diagnose and self-treat. You just need guidance and effort.

Here’s how I remove the trigger points:

-Identify what the trigger point feels like;

-Identify which trigger points to work on based on symptoms;

-Know how to treat actual trigger points. They are not like regular trigger points and won’t respond to a massage;

-Find the spot in the muscle (knot); find the apex of the knot, and put the same pressure on the knot; and move it in a small range motion ½ inch in diameter This allows the milky tissue to release the fluids of muscles.

Trigger points are not commonly known or treated in most patients.
With chronic pain, a patient has latent trigger points which flare up with stress.

Many people take meds, opioids, injections,surgery. I’m not a fan of manipulation (chiropractor). Pain relief from spinal manipulation is short-lived because of the underlying trigger points. If someone has a rotator cuff injury, that means that there are active trigger points.

As a physical therapist for more than a decade, I’ve seen the most ‘aha’ moments when we treat the trigger points. Trigger point treatment allows the patient to come off the meds and avoid surgery.

Surgery is only needed in rare cases such as massive herniated discs pushing on a nerve such as the big trauma after a fall and accidents. But, most people can be helped with time and conservative management.

When too much pressure is applied to a nerve by surrounding tissues such as bones, cartilage, muscles or tendons, a pinched nerve occurs. This pressure disrupts the nerve’s function, causing pain, tingling, numbness or weakness.

Pinched nerves have several key symptoms:
Additional symptoms to those listed above include numbness or weakness in the arms or legs, due to the pinched nerve controlling specific muscles and/or Burning or “hot and cold” sensations.
Physical therapy is often a great option to help deal with the symptoms of a pinched lower back nerve. We will design a program to fit your needs. We can do active or passive treatments depending on the level of your pinched nerve.
Physical therapy works to increase strength and flexibility. Increased strength in your muscles and more flexibility will support the part of the body where the nerve is pinched. Once that starts, the nerve can heal.
We do not often recommend exercising the pinched nerve out on your own. You always have a chance of injuring yourself further when not doing proper healing exercises. However, simply taking it easy on the affected area can be very effective in allowing the nerve to heal. Increasing muscle strength and flexibility will also help to support the area around the pinched nerve.

Opioid usage has ravaged the lives of people all over the United States. The Centers for Disease Control and Prevention estimates that America’s overall economic burden of opioid use/misuse, by itself, costs the nation about $78.5 billion per year. Serious risks exist with long-term opioid use. The CDC recommends safer approaches like physical therapy for the management of most chronic pain.
Safely performing physical therapy exercises during op**te withdrawal may, in some cases, be more effective than traditional treatment methods. Science supports exercise as an excellent counter-withdrawal tool for those choosing to wean themselves from opioid pain management.
Opioids cover up the feeling of pain. At Nau Physical Therapy and Wellness, we treat pain through hands-on care, patient education, and prescribed movement. Our physical therapy offers positive side effects like improved mobility, independence, and wellness.

Opioids are a class of drugs that include:
Hydrocodone (Vicodin).
Oxycodone (OxyContin and Percocet).
Oxymorphone (Opana).

Physical activity boosts the presence of dopamine in the brain. The more we exercise with our individually prescribed exercises, the more dopamine is released. Does it feel like your headache is more than just a headache? Is it affecting your whole body?
You may be suffering from migraine headaches. Physical therapy (PT) can help decrease your pain, improve your mobility, and manage future attacks of headache pain. Sometimes your migraine headache may be coming from your cervical spine (neck), and may be known as a cervicogenic headache.
We can assess your spine, and offer treatments that help improve your mobility and decrease your pain.
Several symptoms of cervicogenic migraine headache may include:
-Pain on one side of your upper neck that radiates to your temple, forehead, or eye
-Reduced range of motion in your neck
-Pain on both sides of the back of your head
-Pain in the back of your head that radiates to one shoulder
-Nausea
-Sensitivity to light or sound
-Dizziness or vertigo
-Pain in one side of your face or cheek
Sudden motions often trigger symptoms of cervicogenic migraine, or they may come on during sustained postures of your neck. The symptoms are often described as dull and non-throbbing, and they may last for several hours to days.
Your healthcare provider may refer you to us at Nau Physical Therapy and Wellness to treat cervicogenic headache once it is diagnosed.”
On your first visit for headaches, we interview you, and take a thorough history of your condition. We will ask you questions about the onset of your pain, symptom behavior, medications tried, and diagnostic studies that have been obtained.
We will also ask about any previous treatments you have tried for your pain, and review your past medical and surgical history.
Elements of the PT evaluation may include:
-Palpation of your neck and skull
-Measures of neck range of motion
-Strength measurements
-Postural assessment
Once the evaluation is completed, we work with you to develop a plan of care and rehab goals for your headaches. Then treatment can begin.
When performing neck exercises for your headaches, you need to move in slow, steady motions and avoid sudden or jerky movements. We may have you add overpressure with your hands to each neck stretch, and the end range position of stretches may be held for up to one minute.
If you have a forward head posture, correcting your posture may be an effective strategy for treatment. Postural correction may include:
-Using a lumbar roll when sitting
-Performing the slouch-overcorrect exercise to find an
optimal posture
- Using a supportive neck pillow when sleeping.”
We also may apply heat or ice to your neck and skull to help decrease pain or inflammation. Heat helps relax tight muscles and improve local circulation. It’s also an effective treatment to do just before performing your neck stretches.
If tight muscles are limiting your neck motion and causing headache pain, we may perform a massage to help improve mobility. We may perform a special technique to loosen the muscles that attach your skull to your neck, allowing for improved motion and decreased nerve irritation.
Mechanical or manual traction helps decrease pain from cervicogenic headache. Traction may decompress your neck's discs and joints and allow for improved motion to occur in your neck.
“We may use one or many different treatments for your cervicogenic headaches,” says Dr. Nau. “Our patients work closely with us so that we determine the effectiveness of each treatment for your headaches.”
“Care for cervicogenic headaches last about four to six weeks,” says Dr. Nau. “You may experience relief within a few days of starting therapy, or your symptoms may remain over the course of weeks.”
“Some people experience continued migraine headache pain for months after starting therapy,” says Dr. Nau. “They use the techniques they learned in PT to help control exacerbations of their symptoms. We give you an idea of how long your specific therapy should last.

Maybe a surgeon told you that you need surgery. But maybe you don’t.

Before you get surgery, see us! Surgeons recommend surgery. To a hammer, the world is a nail! But 3 out of 4 back surgeries have failed outcomes. People don’t get better, they get worse.

Americans spend about $86 billion a year on backaches. That’s on par with what people outlay for cancer treatment.

In the U.S. more than 1,150,000 people go under the knife every year. That’s a rate double that of most developed countries yet the outcomes have been no better. Only about 5% of patients with back pain need surgery.

Back pain is the most common pain we see.
Most back pain is caused by our lifestyle. We sit a lot and create muscle tightening. People overcompensate and that causes compression and more muscle tightening creating herniated discs, lumbar pain, etc.

There is more of a tendency to get certain types of back pain between the ages of 19 & 35. There are more herniated discs which means more liquid, whereas at ages 65 and above, there is more stenosis, or discs are drying out. These issues are too complicated for surgery to help.

Before you get back surgery, try physical therapy. Physical therapy includes soft tissue work, joint manipulation, and fascia. We look at the cause of lower back pain which includes tight and weak muscles. We use acupressure which is a hands-on type of work. There’s also spinal manipulation.

Is your back out of sorts? Aches and pains nagging at your every move?

Maybe a surgeon told you that you need surgery. But maybe you don’t.

Before you get surgery, see us! Surgeons recommend surgery. To a hammer, the world is a nail! But 3 out of 4 back surgeries have failed outcomes. People don’t get better, they get worse.

Americans spend about $86 billion a year on backaches. That’s on par with what people outlay for cancer treatment.

In the U.S. more than 1,150,000 people go under the knife every year. That’s a rate double that of most developed countries yet the outcomes have been no better. Only about 5% of patients with back pain need surgery.

Back pain is the most common pain we see.
Most back pain is caused by our lifestyle. We sit a lot and create muscle tightening. People overcompensate and that causes compression and more muscle tightening creating herniated discs, lumbar pain, etc.

There is more of a tendency to get certain types of back pain between the ages of 19 & 35. There are more herniated discs which means more liquid, whereas at ages 65 and above, there is more stenosis, or discs are drying out. These issues are too complicated for surgery to help.

Before you get back surgery, try physical therapy. Physical therapy includes soft tissue work, joint manipulation, and fascia. We look at the cause of lower back pain which includes tight and weak muscles. We use acupressure which is a hands-on type of work. There’s also spinal manipulation.

So as I like to say, ‘To the hammer, the world is a nail’. Remember that when you speak to a surgeon, that is what they will recommend. You don’t have to be Venus or Serena Williams to get tennis elbow. The term, “tennis elbow” is commonly used for the pain which happens when the tendons on the outer side of your elbow are overworked after repeated use. It's also commonly known as "lateral epicondylitis." It's mainly caused by inflammation of the forearm muscle that is attached to the elbow.

You don’t have to be Venus or Serena Williams to get tennis elbow. The term, “tennis elbow” is commonly used for the pain which happens when the tendons on the outer side of your elbow are overworked after repeated use. It's also commonly known as "lateral epicondylitis." It's mainly caused by inflammation of the forearm muscle that is attached to the elbow. Typically tennis elbow is the result of really repetitive movements such as in tennis, racketball, and even in jobs like painting and construction. Anyone involved in any activity that involves repeatedly moving your wrist and arm, and using their joints and muscles ineffectively, could be affected by this condition.

Common symptoms can include pain and weakness on the outside of the patient’s elbow, which radiates to the wrist and forearm. We work with our patients to identify why their injury might have happened, and work out some exercises that can help them use their joints more effectively, and take the strain off them.

We work with our patients, not only improving the motion of the joint, but also, in time, reducing and eventually eliminating the pain of the injury. Exercises we might suggest are aimed at improving blood flow to the tendons, which typically receive much less oxygen than muscles around them, or might be focused on several other things.

Here are some examples:
Exercises that involve stretching and strengthening of the muscles;
Massaging using essential oils or ice to reduce inflammation;
Stimulating the muscles to improve blood flow;
Using straps or braces to provide additional support until your elbow has healed sufficiently.
With physical therapy, you can improve the strength and flexibility of the muscles in your forearm which means the likely-hood of being bothered by tennis elbow again are slim. Similarly, physical therapy for tennis elbow can help improve the blood flow to the muscles in your forearm which will help supply essential blood and oxygen to the muscles as fast as possible.”
