Job Title: Caregiver Coach

Job Summary:
We are seeking an experienced Caregiver Coach to provide guidance and support to individuals who are caring for elderly or disabled family members. The Caregiver Coach will work with clients to assess their needs, develop care plans, and provide ongoing support and education to help them provide the best possible care for their loved ones.

Key Responsibilities:
• Conduct initial assessments of clients' needs and develop individualized care plans
• Provide ongoing coaching and support to clients, including regular check-ins and follow-up appointments
• Educate clients on best practices for providing care, including medication management, nutrition, and hygiene
• Connect clients with resources and support services in the community, such as home health aides, respite care, and support groups
• Monitor clients' progress and adjust care plans as needed
• Maintain accurate records of client interactions and progress

Requirements:
• Bachelor's degree in nursing, social work, or a related field
• At least 3 years of experience working with elderly or disabled individuals and their families
• Strong communication and interpersonal skills
• Ability to work independently and manage a caseload of clients
• Knowledge of community resources and support services
• Experience with care planning and coordination
• Ability to maintain confidentiality and comply with ethical and professional standards

Preferred Qualifications:
• Certification as a Caregiver Coach or equivalent
• Experience working in a healthcare or social services setting
• Knowledge of chronic illness and geriatric care management
• Bilingual proficiency in English and another language
