Damian Lillard is back with the Portland Trail Blazers and we love a good comeback story! Just like our patients at Porthaven Post Acute, Dame knows it takes discipline, grit, and the right team to get back to your best. While he’s busy draining threes, we’re helping folks put in the work right here in our rehab gym. Our therapists are some of the best in the PNW — so Dame, swing by and get a real workout in with our top-of-the-line therapy team. Who knows, we might even put you on the stationary bike for some full-court cardio. 😉 Around here, every step forward is Dame Time.

At Porthaven Post Acute PACS, we pause today to honor the lives lost, the families forever changed, and the heroes who answered the call on September 11th — many of whom still live among us and continue to inspire us with their courage.

This day reminds us that the strongest communities are built on unity, kindness, and service. In their memory, let’s each take a moment to do something small but meaningful — lend a hand, share a kind word, or support a neighbor in need.

Together, we remember. Together, we heal. Together, we serve.

🌟 Staff Spotlight Porthaven Post Acute PACS: Chelsea Richards 🌟

We are beyond lucky to have Chelsea Richards as our Assistant Director of Nursing and Infection Preventionist Nurse. Chelsea first joined us as a Resident Care Manager, where her dedication, clinical expertise, and compassion quickly stood out. It wasn’t long before she earned her well-deserved promotion — and she’s been raising the bar ever since.

Chelsea is the definition of a leader who isn’t afraid to roll up her sleeves. She’s right there on the floor when we’re short, supporting her team and caring for patients. From monitoring every infection to overseeing wound care and ensuring our trach patients receive the specialized support they need, Chelsea brings both skill and heart to everything she does.

Her ability to balance big-picture infection prevention with hands-on, bedside nursing makes her truly one of a kind. Around here, we call her the Tom Brady of nursing 🏆 — dependable, unstoppable, and the ultimate team player.

Outside of work, Chelsea lives life to the fullest — snowboarding, working out, and spending quality time with her beautiful family. And while she may be a loyal Seattle Seahawks fan 🏈 (we try not to hold that against her 😉), her dedication to our patients and staff is something we can all cheer for.

Chelsea, thank you for the compassion, energy, and leadership you bring every single day. Our team, our residents, and our families are better because of you. 💙

✨ Celebrating a Milestone at Porthaven Post Acute ✨

We are thrilled to share that our patient, Jean Bishop, has successfully been decannulated from her tracheostomy and is preparing for discharge home tomorrow.

This achievement would not have been possible without the expertise and dedication of our Director of Pulmonary Services, Chris Woods, who carefully guided Jean through every stage of her respiratory care, and our outstanding therapy team, who worked tirelessly to help her regain her prior level of independence.

Moments like these remind us why we do what we do. Congratulations, Jean — we are so proud of your progress, and grateful for the incredible team who made this milestone possible. 💙

✨ Please meet our Director of Therapy, Nate Rasmussen ✨

Nate holds a Master’s degree in Occupational Therapy from Saint Augustine University. Originally from Tucson, Arizona, he recently moved to Portland from Sacramento, California.

With extensive expertise in rehabilitation and complex recovery care, Nate specializes in tracheostomy management, bariatric care, and post-operative orthopedic rehabilitation. He takes a patient-centered approach, working closely with each individual to create tailored care plans that support both medical and functional goals. His focus is always on restoring independence and helping patients transition safely back home or to their prior level of care.

As a leader, Nate ensures our therapy team delivers evidence-based treatment, compassionate support, and measurable outcomes, making rehabilitation a collaborative and empowering process for patients and families alike.

Beyond his professional accomplishments, Nate enjoys hiking and paddle boarding, and at home he spends time with his wife and Bernedoodle, Betty.

We are proud to have Nate guiding our exceptional therapy team and bringing his passion, skill, and dedication to the patients we serve. 💙

🎪✨ Carnival Day Fun at Porthaven Post Acute. What a joyful day we had as our amazing Activity Directors brought the carnival spirit right to our patients! From laughter to games, the smiles were endless. Even our Administrator joined in on the fun—proof that a little carnival magic brings everyone together. 🥳🎟️🍿

Big thank you to our team for creating such special memories for our patients—it’s days like this that remind us how much joy community and connection can bring. ❤️
