Consent and Policies
Consent: I agree and consent to assessment and treatment. I understand that all information that I provide will be kept confidential unless required by law. I understand and consent that my medical information may be shared by the various care providers involved in my care and treatment. Cancellation: Please provide at least 24 hours notice if you need to reschedule or cancel a treatment. If a client cancels within the 24 hour window, the client will be charged the full amount of the appointment cost. This must be paid prior to scheduling any future appointments. If a client fails to cancel within 24 hours multiple times (2 or more), they may be asked to pre-pay for future services. Late arrival: I regret that late arrivals may not receive extension of scheduled appointments. In special cases, and when my schedule will allow, I may be able to accommodate a partial or full appointment. This will be at my discretion and only with proper, advanced notification of your late arrival. The original reservation fee will be charged. No show: I understand that unanticipated events occur in everyoneโs life. Unforeseen events such as car problems, business meetings and childrenโs illnesses, are just a few reasons why one might consider canceling a massage appointment. However, I ask that you call if you cannot keep your appointment. Clients who fail to show for appointments and do not make contact will be invoiced for the appointment and blocked from future bookings until the invoice is paid. You may also be asked to pre-pay for future services. Scope of practice: I am a licensed professional and held to the highest standards by the American Massage Therapy Association (AMTA). I do not diagnose or prescribe for medical conditions. I may refer you to a medical doctor for diagnosis. My services are not intended as an alternative for proper medical attention for specific conditions. Boundaries: I am happy to adjust pressure, temperature, musical volume, or work longer on an area or move on if you request it. The client may choose to: leave on as much clothing as needed for comfort, refuse any massage methods or stop massage at any time. The client will always be modestly draped. Only the area being massaged will be undraped. Sexual interaction or discussion of any kind between the client and the massage therapist is NEVER appropriate. The session will be terminated immediately and the client will be responsible for full payment. Existing and new medical conditions: It is the responsibility of the client to keep me informed of any medical treatment currently being taken, and to provide written permission from the physician, chiropractor, physical therapist, etc., that the massage may be continued. The client must also keep the massage therapist informed of any changes in health conditions. For clients undergoing chemo and radiation therapies โ Please note that we require a doctorโs note that states the doctor is aware of, and agrees to, the desired treatment. Underage clients: Clients under the age of 18 must have a signed authorization form by their parent or guardian. At the time of the appointment, the parent or guardian will also accompany the minor in the treatment room while discussion of the treatment is taking place. At that time it will be at the discretion of parent and minor if the parent shall stay in the room or stay in the waiting area for the duration of the massage.
