This opportunity allowed us to learn, collaborate, and bring back fresh ideas on how to better share the value of home care and connect more effectively with families in our community.

Full day with some incredible people💚
Connected with Home Health Community Liaison Chelsea to discuss Home Health and In Home Care dual discharge benefits, then celebrated the amazing Case Management & Social Work teams at Holy Family Hospital over Olive Garden while talking about continued collaboration for positive patient outcomes and even lower readmission rates (already below the national average 👏🏻)
Finished the day at Life Care Post Falls spreading some St. Patrick’s cheer with Lucky Lime Leprechaun punch for patients, residents & staff ☘️🌈 Grateful for strong partnerships and teams dedicated to making a difference💚

If you have one, don’t keep it locked away in a safe. Make sure your loved ones know your wishes and have copies available if they’re ever needed.

We had so much fun visiting our caregivers and delivering gifts to celebrate the incredible work they do each day 💙

We are so grateful for their commitment and longevity with our team — far beyond industry standards — and for the compassionate care they provide to their patients. From specialty care (post-surgical, stroke recovery, Parkinson’s, Alzheimer’s, dementia, and end-of-life care) to respite support, transportation, companionship, light housekeeping, meal prep, errands, and assistance with daily activities (like bathing, dressing, medication reminders, mobility and more), their dedication keeps clients safe, engaged, and supported at home.
