The office is completely closed. Medical records are still being worked on to transfer or download to USBs. Please mail records requests to PO Box 1072, Poth, TX 78147. Page one is all that is needed. Each patient must have their own form. The completed document can be faxed to (210) 569-6488 or can be mailed.

****Medical Records****

We understand that everyone wants their medical records as soon as possible and we want you to have them as well. Faxes have been failing because some of the files are so large.

Going forward FAXES will be the LAST 2 YEARS of medical records.
If you want ALL the medical records please mail a USB drive to our PO BOX or come into the office and drop it off in the box in the lobby.

Please include patient names with their date of birth. If we do not have a medical records release. Please include one for each patient. Please indicate if you want to pick up the USB or have us mail it back to you with address written down.

Release forms will be available in the lobby as well.

****OFFICE HOURS****

Starting Monday October 6, 2025, Mrs. Ryan will be in the office Monday-Thursday from 10am to 2pm.

Please try to limit interruptions as she will be working on medical records. The front reception window will remained closed.

Release forms for medical records will be in the lobby along with a USB drive drop off box.

It's hard to believe that the last clinical day of Ryan Pediatrics is today. While the clinic is closed we are still hard at work.

We are still working hard at ensuring all the medical records get released.

To obtain medical records you will either need to fill out and sign our release form or one from the new practice. Please fax or hand in the release form. It cannot be sent via e-mail due to personal information on the form.

Faxing to the new practice is free. Sometimes the faxes are failing due to the large file size.

You may also bring in your own USB drive and we can transfer the files to that and you may pick it up. This will also be free of charge as we are not using ink or paper.

If you would like the records printed please be aware that there is a fee; $1.00/page for the first 50 pages and $0.50/page after that.

Please message us to make sure someone is in office to hand out the form or collect a USB drive.

We are deeply saddened to announce that Ryan Pediatrics will be permanently closing as of September 30, 2025. Mrs. Ryan has been blessed through the years to have support through the community and to have their families trust in her to care for their children. It is with a heavy heart that Mrs. Ryan has made the decision to close the practice.
Please understand we are going to be doing our best to fit patients in, but the schedule is extremely busy.
If you need immunization records, please contact us as soon as possible.
