Health Services of Northern New York is seeking a Registered Nurse Case Manager to support Canton and the surrounding areas.

This position will be responsible for the management of client care in the home setting. The Registered Nurse Case Manager will perform nursing assessments, coordinate care with physicians, and have oversite of additional discipline involvement with each client.

This position will require travel to and from patient homes throughout their designated area.

Minimum starting salary for this position is $71,000. Registered Nurse Case Managers will also enjoy benefits of health, dental and vision insurance, 401k, flexible scheduling and more!
