With five convenient locations throughout Pottstown, Barto, and Boyertown, we want to ensure that you can access the care you want, when you need it. We offer a wide range of services, including Medical, Dental, Vision, Physical Therapy, Podiatry, Women's Health - OB/GYN, Behavioral Health, and more! We accept most major Commercial, Medicare and Medicare supplement plans, Fee for Service Medicaid and Managed Care Medicaid plans and provides a health care discount for eligible uninsured and underinsured patients. For qualified patients receiving a 100% healthcare discount, the nominal fee for a Medical/Behavioral Health/Vision visit is $25, $40 for a Dental visit and $7 for a Prescription! CHDC can offer the health care discount because we receive annual funding from our Federal partner, Health Resources and Services Administration (HRSA) since 2012 as a designated Federally Qualified Health Center. Are You Exempt from SNAP Work Reporting Requirements? SNAP recipients must report at least 20 hours of work, education, training, and/or volunteering per week (or 80 hours per month) to receive benefits. You may be exempt from these new work and reporting requirements. To learn more about these requirements, visit dhs.pa.gov/work.

CHDC’s Case Managers are community partners with the PA Department of Public Welfare and can identify patients who are at risk and connect them with the support and services they need. Our team provides assistance to help ensure you receive the benefits that are right for you. Not sure if you qualify? Talk to your CHDC provider about Case Management during your next visit.

CHDC strongly advocates for disease prevention through immunization. Pediatric providers at CHDC stand ready to partner with you to provide your family with evidence based medical care by following the vaccine recommendations put forth by the American Academy of Pediatrics (AAP). AAP vaccine recommendations are based on scientific evidence proving these vaccines are a child’s best defense against preventable diseases and respiratory viruses.

As medical professionals, we feel very strongly that vaccinating your child on schedule with currently available vaccines is absolutely the right thing to do to protect all children and young adults. Please feel free to discuss any questions or concerns you may have about vaccines with your CHDC provider.
