Step 1: Notice the triggering situation. Step 2: Write the automatic thought. Step 3: Rate the emotion (0–100). Step 4: Look for evidence for and against the thought. Step 5: Create a balanced alternative and re-rate your emotion.
Client vignette: A teen shared she felt ‘‘everyone is judging me’’ before a presentation. Using the thought-record she found evidence that classmates had been supportive, created a balanced thought—“Some people may notice me, but I can handle it”—and her anxiety dropped from 80 to 40. Over a few weeks she felt more confident speaking up.
