We are seeking a skilled and compassionate Physical Therapist (PT) to join our growing team.

Position Summary:
As a Home Health Physical Therapist, you will provide direct patient care and develop individualized treatment plans that help patients achieve their mobility and rehabilitation goals in the home setting.

Key Responsibilities:
• Evaluate and assess patients’ functional abilities and home environments
• Develop and implement personalized physical therapy plans of care
• Educate patients and caregivers on safety, mobility, and exercises
• Collaborate with physicians, nurses, and other clinicians as part of the interdisciplinary team
• Maintain accurate, timely documentation in accordance with agency and Medicare standards
• Monitor progress and adjust interventions as needed to meet goals
• Ensure compliance with HIPAA, CMS, and agency policies

Qualifications:
• Current Physical Therapist license in the state of Wyoming
• Graduation from an accredited Physical Therapy program
• Prior home health or geriatrics experience preferred, but not required
• Valid driver’s license, reliable transportation, and auto insurance
• Strong clinical, communication, and time management skills
• Ability to work independently and manage a flexible caseload
