We have Hawthorne Inn Supportive Living and Skilled Nursing, our Bounce Back Rehabilitation Program, AJ's Fitness Center and Wellness Program, Outpatient Therapy Services, and Memory Lane - Fitness for the Mind Dementia Care. Whatever your need, Liberty Village is available to help!

We're hiring a MDS & Care Plan Coordinator! 📋✨

Our MDS & Care Plan Coordinator is responsible for coordinating the completion of the RAI process and Care Plans.

We offer benefits such as:
• Group Health Insurance
• 401 K Plan (company match to 5%)
• Holiday Pay (double time)
• Employee Time off (up to 5 weeks a year)
• Flex Spending Plan

All LPNs & RNs must be licensed in Illinois.
