Our integrated team of home health and hospice experts are trained to carefully consider all of the challenges that their patients and their loved ones may face, including their medical, psychological, spiritual, financial and social needs. Our integrated team refuses to let patients “fall through the cracks.” Our new, innovative Frontier Integrated Care Solutions (FICS) palliative program provides support and timely resources for all of our patients at all stages.

Our collaboration of care is seamless between our Home Health and Hospice team. Whether you or a loved one is recovering after a surgery, a medication change, short stay in a rehabilitation facility, or a recent hospitalization. Our team of Home Health professionals, that consists of Nurses, Physical Therapist, Occupational therapist, Certified nurses aids, and Medical Social Workers, knows how to get you on your way back to maintaining the life you lived. They can also give you the right tools to empower you to make the decision on when it is time to look towards comfort measures with our Hospice Team. Our way of guiding you and your family member, so it is not a scary time in your life, rather a sigh of relief knowing someone with end of life expertise is there to guide you in next chapter, focusing on mind, body spirit.

Anyone can call our team today. It is FREE for a consultation, and we take it from there by working with your physician to obtain orders, etc. Home Health and Hospice services are paid for by your insurance provider so there is no out-of-pocket expense to you. (Call for details).
