PATIENT NAVIGATOR:
RMC is looking to hire a Patient Navigator to work full-time at our main clinic location in Rainelle.
Quality Requirements:
Essential Duties and Responsibilities:
· Responsible for assisting in and providing safe, consistent, and appropriate care within the scope of the established RMC
practice guidelines.
· Assists quality staff with care gap management and scheduling.
· Patient monitoring and outreach, conducts regular checks on the Azara platform to manage care gap lists and performs outreach
for scheduling purposes.
· Encourages patients to complete preventative services and works with the patient to ensure scheduling and follow up is completed.
· Assists Quality Director in providing educational materials to patients as assigned.
· Assists with all care gaps with a special focus on CRC screenings and rates
· Attends meetings/trainings assigned by DOQ, Chief of Nursing, or DEAP Coordinator
· Be familiar with appointment systems and eligibility requirements for various available programs.
· Communicate closely with Provider Staff, Nursing Staff, Quality Staff, and DEAP staff.
DEAP REQUIREMENTS, include but are not limited to:
· Assist with compilation of patient case management information through the utilization of EHR, Diabetes Case Management
Systems, and manual chart audits of patients with diabetes.
· Contributes to quality assurance measures and the accuracy of diabetes patient information through effective chart and
information maintenance and management.
· Assist in the planning and implementation of diabetes-related functions, activities, classes, screenings, workshops, etc. in the
community and clinic.
· Assist with development and implementation of a diabetes community assessment system including the compilation and delivery of related information to relevant auditing entities.
· Provides advocacy and education services information to clients and facilitates access to needed health services that may be
available within the program.
· Encourages and solicits participation in the Diabetes Education Program, both within the clinic and outside healthcare providers.
· Submit required Prior Authorizations to appropriate insurance companies for needed diabetes related equipment and medications.
· Verify patient insurance accuracy and benefit detail.
· Develop and maintain patient tracking data for demographic and outcome purposes.
· Attend all required staff meetings.
· Assists with referrals as assigned
· Must have and maintain knowledge of and interest in community health and primary care practice environments.
· Must have the ability to work collaboratively with diverse and varied individuals, teams, populations both
internally and externally.
· Must have and maintain a solid problem identification, analysis and resolution skills.
· High level of professional acumen, initiative and time management skills.
· Perform related duties as assigned by supervisor
· Maintain compliance with all company policies and procedures
EDUCATION AND/OR WORK EXPERIENCE REQUIREMENTS: Excellent verbal and written communication skills, including ability to effectively communicate with internal and external customers · Proficient in EMR/EHR System · Must be able to work under pressure and meet deadlines, while maintaining a positive attitude and providing exemplary customer service · Possess basic computer and database software operations knowledge and demonstrate working knowledge of routine office hardware and equipment. · Ability to demonstrate basic levels of computer literacy, with functional understanding of Microsoft Windows Operating Systems in an office setting. · Demonstrate the ability to maintain accurate records and prepare clear and accurate reports for informational, auditing, and operational use, including capacities for attention to detail and for reviewing and verifying accuracy of data. · Demonstrate professional competency in office and healthcare abilities, including the maintenance of client confidentiality. · Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices · Possess the ability to perform well in group problem-solving situations and use of reason even when dealing with highly emotional topics. · Possess the ability to become knowledgeable of health needs and services provided to the patient demographic being served. · Possess the ability to deliver effective training and presentations on technical subjects to large and/or small groups of various educational and cultural backgrounds · Must be familiar with entering data into a healthcare database program (EHR) · Responsible for tracking quality measures and create spreadsheets used for tracking purposes for DEAP. · High School Diploma/GED · Experience with referrals/EHR/Tracking/Quality Measures preferred.
