Some women seeking a breast lift also want fuller-looking breasts, but they do not want breast implants. An auto-augmentation mastopexy is designed to use lower pole tissue that would normally be discarded to create upper pole fullness that makes the breast look more youthful and perky. The breast looks fuller, but they feel lighter and more comfortable for a woman because the breasts are in a higher, more youthful position. In this operation, the lower pole tissue, instead of being removed, is mobilized but left attached to the chest wall to provide a blood supply. That tissue is then tunneled behind the breast into the upper pole, where it is sutured to the pectoralis fascia to hold it in place. This is an excellent technique for creating lasting upper pole fullness in patients who prefer not to have an implant or do not want to have a breast volume as large as what would be created by an implant. Dr. Law has been seeing patients worldwide since he started performing this procedure in 2012. The procedure creates beautiful results that, on the OR table, look as though an implant might have been placed. The long-term result is natural upper pole fullness that matches the rest of the breast proportions. The surgery is so successful and provides women with so much satisfaction that Dr. Law no longer offers breast implants as part of a lift and only uses the natural tissue of the breast, and possibly some fat grafting if needed to provide a lift and a natural-looking breast profile to his patients.

Gynecomastia affects men in many different ways, and each individual may require a customized approach to treatment. For some, a full excision is needed, which can be performed as a staged procedure or in a single surgery. The advantage of a staged procedure is that skin contraction often eliminates the need for incisions on the chest wall. On the other hand, some prefer a single procedure where incisions are made. This patient is shown three months post-surgery, and while scars continue to fade over time, many become almost invisible within a year or two. The transformation after gynecomastia surgery with Dr. Law is truly remarkable. For men with stage four gynecomastia, asymmetry, or darker skin, having a surgeon with expertise in creating ideal contours and minimizing scarring is crucial.
