💜 Honoring the past, serving the present, and building for the future. Tomorrow at the Duke Caregivers Conference, our Transitions LifeCare team is sharing pieces of our story—old photos, memories from 1979 to now, and the milestones that shaped who we are today.

💜 Today is National Grief Awareness Day. At Transitions LifeCare, we honor this day by reminding our community that grief looks different for everyone—and support is available.

We offer a variety of programs like Conversations on Grief, Expressive Arts Workshops, and our Support Over Solitude group for children. 👉 Be sure to check the details before registering, including the week’s topic for Conversations and the specific art activity planned for Expressive Arts.

This sweet kitten found her forever home with one of our Spiritual Care Counselors, bringing comfort and peace of mind to a patient knowing their furry family was loved and cared for. 💜 At Transitions LifeCare, compassion extends beyond our patients—it touches every member of the family. Stay tuned for more stories of hope and adoption that remind us love comes in all shapes, sizes, and paw prints.
