For several years Hospice of Wake County has offered more than “hospice” care and has served patients well beyond “Wake County.” Though we changed our name to Transitions LifeCare, our mission and vision are unchanged and we remain a local, non-profit serving patients regardless of their ability to pay. The core brand name

“Hospice of Wake County” no longer accurately reflected all we do. We’re not just hospice. And we serve more than Wake County. Our service territory extends well beyond Wake County so we help more patients and families in Durham, Johnston, Franklin, Harnett, Wilson, and Nash counties, eastern Chatham County and in the communities of Chapel Hill and Carrboro in Orange County. Rest assured our expertise, compassion and commitment to helping families facing a serious or terminal illness remain the same. We are still a non-profit organization with a local board of directors. We have not been bought by a large, for-profit corporation. Going forward, Transitions LifeCare is our single brand name and our service lines have corresponding names: Transitions HospiceCare, Transitions Kids, Transitions PalliativeCare, and Transitions GriefCare.

At Transitions LifeCare, we believe caring for someone means caring for the whole person. Emotional and mental well‑being are just as essential as physical comfort and clinical care. Through counseling, social work, spiritual care, grief services, and compassionate guidance, our teams are here to support patients and families through every part of the journey.

During Mental Health Awareness Month, we’re reminded that no one should have to navigate serious illness, caregiving, or grief alone. 💜

We are honored to join our partners from The Women's Center and WakeMed Health & Hospitals in supporting the development of the new Wake Respite center, which will include two dedicated hospice rooms. Our leadership teams met recently to tour the future facility – giving us a powerful look at how this project will expand care for neighbors who need our support. This will help fill a vital gap for individuals experiencing homelessness who need a safe place to heal, and we are grateful to be part of a community working together to make meaningful change.
