Transitions LifeCare - NC Founded as Hospice of Wake County in 1979. Serves 8 counties including Wake. Oldest, largest and most experienced end-of-life organization in the area. Why?

For several years Hospice of Wake County has offered more than “hospice” care and has served patients well beyond “Wake County.” Though we changed our name to Transitions LifeCare, our mission and vision are unchanged and we remain a local, non-profit serving patients regardless of their ability to pay. The core brand name “Hospice of Wake County” no longer accurately reflected all we do. We’re not just hospice. And we serve more than Wake County. Our service territory extends well beyond Wake County so we help more patients and families in Durham, Johnston, Franklin, Harnett, Wilson, and Nash counties, eastern Chatham County and in the communities of Chapel Hill and Carrboro in Orange County. Rest assured our expertise, compassion and commitment to helping families facing a serious or terminal illness remain the same. We are still a non-profit organization with a local board of directors. We have not been bought by a large, for-profit corporation. Going forward, Transitions LifeCare is our single brand name and our service lines have corresponding names: Transitions HospiceCare, Transitions Kids, Transitions PalliativeCare, and Transitions GriefCare.

We’re proud to share that Keri Burnette, Provider Relations Representative at Transitions LifeCare, recently served as a panelist for the Osher Lifelong Learning Institute at North Carolina State University as part of the program, “Plan for Aging: Live Well Today and Plan Wisely for Tomorrow.” 💬📝

Keri presented “When the Focus Changes, Care Should Too,” offering clear, practical guidance on when to engage hospice and palliative care—an often misunderstood but critically important part of the care journey.

Thank you to Pathfinder Wealth Consulting for inviting us to be a part of this important conversation. The room was full and participants were highly engaged throughout the discussion, reflecting both the importance of planning for serious illness and Keri’s ability to connect and educate. We’re grateful for the way she continues to show up for our community—helping individuals and families navigate complex decisions with clarity and compassion. 💜🫶

Hosted by the American Nephrology Nurses Association, this conference brings together nephrology professionals to learn, connect, and stay at the forefront of patient care.

His session, “Getting into the Weeds with Medical Cannabis and Hemp,” tackled a topic we hear about every day from patients and families—bringing clarity, education, and real-world application to an evolving space in symptom management.

We’re grateful for the opportunity to be part of this important conversation. At Transitions LifeCare, we’re committed to meeting patients where they are and exploring thoughtful, individualized approaches to care—and this is a great example of that in action.
