On January 13 & 14, our campus will once again glow with hundreds of luminaries as we honor the lives of those who have died and hold space for all who grieve. Lights of Remembrance offers a peaceful, self-guided walk through illuminated pathways, inviting reflection, connection, and comfort in community. Join us! 💜

After catering the Transitions Kids 10th Anniversary celebration last September, their team was deeply moved by the work we do and wanted to give back this holiday season. Thank you for showing up with such heart and helping care for the families we serve. 👏

There’s something special about slowing down over tea, homemade treats, and fresh flowers. Thanks to our wonderful volunteers, our Tea Time tradition creates a cozy space for connection, conversation, and comfort for our patients and families. These moments are what make our Hospice Home feel like home. 💜🏡

Kicking off the new year with some meaningful news 🎉 We’re grateful to Searstone and Lutheran Services Carolinas for awarding Transitions LifeCare a $111,500 grant to support Wake County indigent seniors. This investment helps ensure compassionate, high-quality hospice care remains accessible to some of our community’s most vulnerable neighbors. 💜

Our Holiday Music Bingo series was a hit! 🎄🎶 A month filled with smiles, laughter, festive prizes, and moments of real connection. These photos from Granville County Senior Services capture just a glimpse of the joy—so much so, they insisted on jumping in the picture! Proof that a little music and community can go a long way during the holidays. ❤️

This Feel Good Friday, we’re grateful to Woodland Terrace Kisco for continuing a tradition that makes a difference. Their annual Stuff the Bus toy drive was delivered to our campus last week, and we can’t wait to share it with our Transitions Kids families this holiday season. Thank you to everyone who helped make this possible! ❤️🎅🕯️

Holiday cheer, handmade decor, and a table full of homemade dishes — all thanks to the Raleigh Garden Club. Each year, they lovingly decorate our Hospice Home and admin building and host a holiday potluck that brings warmth, comfort, and connection to our team. We’re so grateful for their continued support and the heart they pour into everything they do. ❤️🎉🎄
