Our Senior Disconnect gathering last week was exactly what we hoped for — a chance to slow down, connect, and bring a little extra warmth to the season. From the blessing-of-the-hands ceremony to crafting holiday stockings and baskets for our Hospice Home families, every moment felt meaningful.

There’s something special about watching our Transitions Kids patients and families light up at our annual Fall Fest. 🍂✨ With help from over 20 dedicated volunteers and our TK teams and their families, the day was filled with laughter, games, and moments our kids will remember.

We’re grateful to McLean Farms Nursery & Ice Cream Shop for donating pumpkins and to Sunrise Community Farm Center for bringing therapy bunnies and a horseback-riding raffle — your generosity helped make the day brighter for our kids and families. 💜🧡

We had a wonderful time connecting with the Wilson community at this year’s Whirligig Festival! 💜

This well-attended event gave us a meaningful opportunity to share Transitions LifeCare’s mission and make an impact—helping more families learn about the compassionate care and support available across eastern North Carolina. We’re so grateful to everyone who stopped by our booth and helped us spread the word about the importance of compassionate end-of-life care.

Transitions LifeCare’s Community Engagement team recently visited The Stratford Assisted Living in Chapel Hill to lead an educational discussion with their leadership team.

Our Associate Director of Education, Dwan Kelsey (pictured right), facilitated a meaningful conversation on recognizing the differences between confusion, delirium, and dementia. Also pictured is Tawana Coleman, Clinical Manager at The Stratford.
