Person Street Pharmacy is currently enrolling patients to the Remote Patient Monitoring (RPM) program for High Blood Pressure.
To qualify patients must be enrolled in Medicaid or Medicare (not Humana), have high blood pressure, and take BP medications.
Everything is covered with no costs to you.

How does it work?
-Schedule an initial one time 30 min appointment at the pharmacy with the pharmacist, Maresa Roney.
-Receive your free Blood pressure cuff
-Take your blood pressure at least 16 times per month at home, your results are automatically sent to the pharmacy.
-If your daily results raise any concerns, the pharmacist will contact you and work with your primary care doctor.
