Breast augmentation 🦋 1 year post-op.

✨At age 69, this healthy woman decided to go for it and live her best life! Why not? She was and still is in excellent health!

✨For her breast augmentation, we chose to go submuscular as her sun-damaged skin was very thin. Going under the muscle would help better conceal her implants and give a more natural look.

✨One challenge we encountered was rib cage asymmetry. This is actually fairly common. In this case, the left ribs were curved inwardly, whereas the right ribs were more curved outward.

✨The solution here was to use implants with the same base diameter, but with different projections. I placed a Natrelle Soft Touch full profile implant on the left side (520cc) and a moderate profile implant on the right side (405cc).

✨One year later, she is indeed living her best life! Happy patient, happy surgeon! 💁🏽‍♀️

✨IMPLANT BREAST RECONSTRUCTION AND RADIATION THERAPY ✨

• This 41 yo patient was diagnosed with right breast cancer. She had undergone a breast reduction in her 20s. Because of a strong family history of breast cancer, she elected to undergo bilateral mastectomies. On final analysis, she was found to have metastasis to the lymph nodes in her right armpit.

ℝ𝕒𝕕𝕚𝕒𝕥𝕚𝕠𝕟 𝕥𝕙𝕖𝕣𝕒𝕡𝕪 was recommended to the right breast and armpit after surgery for this patient. This always brings several challenges when it comes to breast reconstruction with implants. 𝗕𝗲𝗰𝗮𝘂𝘀𝗲 𝗿𝗮𝗱𝗶𝗮𝘁𝗶𝗼𝗻 𝗶𝘀 𝗹𝗶𝗸𝗲 𝗮 𝗯𝘂𝗿𝗻 𝘁𝗼 𝘁𝗵𝗲 𝘀𝗸𝗶𝗻 𝗮𝗻𝗱 𝘂𝗻𝗱𝗲𝗿𝗹𝘆𝗶𝗻𝗴 𝘁𝗶𝘀𝘀𝘂𝗲𝘀, 𝘁𝗵𝗲𝗿𝗲 𝗶𝘀 𝗮 𝗹𝗼𝘁 𝗼𝗳 𝘀𝗰𝗮𝗿 𝘁𝗶𝘀𝘀𝘂𝗲 𝗳𝗼𝗿𝗺𝗮𝘁𝗶𝗼𝗻, 𝘁𝗵𝗲 𝘀𝗸𝗶𝗻 𝘁𝗶𝗴𝗵𝘁𝗲𝗻𝘀 𝗮𝗿𝗼𝘂𝗻𝗱 𝘁𝗵𝗲 𝗶𝗺𝗽𝗹𝗮𝗻𝘁, 𝗼𝗳𝘁𝗲𝗻 𝗰𝗮𝘂𝘀𝗶𝗻𝗴 𝘁𝗵𝗲 𝗶𝗺𝗽𝗹𝗮𝗻𝘁 𝘁𝗼 𝘀𝗶𝘁 𝗵𝗶𝗴𝗵𝗲𝗿 𝗼𝗻 𝘁𝗵𝗲 𝗰𝗵𝗲𝘀𝘁. 𝗔𝗹𝘀𝗼, 𝘁𝗵𝗲 𝘀𝗸𝗶𝗻 𝗼𝗳𝘁𝗲𝗻 𝗱𝗮𝗿𝗸𝗲𝗻𝘀 𝗳𝗿𝗼𝗺 𝘁𝗵𝗲 𝗯𝘂𝗿𝗻. 𝗔𝗹𝗹 𝗼𝗳 𝘁𝗵𝗶𝘀 𝘁𝘆𝗽𝗶𝗰𝗮𝗹𝗹𝘆 𝗹𝗲𝗮𝗱𝘀 𝘁𝗼 𝗯𝗿𝗲𝗮𝘀𝘁 𝗮𝘀𝘆𝗺𝗺𝗲𝘁𝗿𝘆 𝗱𝗲𝘀𝗽𝗶𝘁𝗲 𝗮 𝗽𝗹𝗮𝘀𝘁𝗶𝗰 𝘀𝘂𝗿𝗴𝗲𝗼𝗻’𝘀 𝗯𝗲𝘀𝘁 𝗲𝗳𝗳𝗼𝗿𝘁𝘀.

✨To address some of the challenges, my preferred approach is to proceed with 𝕡𝕣𝕖𝕡𝕖𝕔𝕥𝕠𝕣𝕒𝕝 𝔻𝕀ℝ𝔼ℂ𝕋-𝕋𝕆-𝕀𝕄ℙ𝕃𝔸ℕ𝕋 breast reconstruction with acellular dermal matrix in this context.

✨This is exactly what we did here and I am quite pleased with this result! I used 520 mL full profile silicone breast implants. The result shown here is 1.5 years after surgery and nearly 1 year after completing radiation.

✨Before and 12 weeks after of this 30 yo woman with BRCA1 gene mutation. She underwent nipple-sparing mastectomies and breast reconstruction.

✨This was a challenging reconstruction because of the presence of **tuberous breast deformity**. This is a congenital breast finding that can occur in one or both breasts. It is thought to be caused by thick connective tissue that restricts breast development. The most common features are:
• Narrow, constricted breast
• Herniation of the breast tissue
• Protrusion of the ar**la
• Downward-pointing ni***es and breasts
• Higher than normal breast fold
• Narrow base of the chest wall
• Wide spacing between the breasts

✨To achieve this result, I planned this surgery in two-stages:
• First, I placed tissue expanders at the time of the mastectomy. This allowed me to make adjustments after surgery and expand the breast skin to increase her breast size.
• Once we reached the size she was happy with, I planned the second surgery to place the permanent implants. I also performed fat grafting at that time.

✨Before and 12 weeks after Breast Reduction on this teenager.

✨I performed a breast reduction and removed a total of 3.5 lbs off her chest! Talk about a major difference!

✨Breast reduction is among the plastic surgery procedures that has the highest patient satisfaction. Why? Because the surgery is both functional and aesthetic in nature. It helps relieve neck, shoulder and back pain, and helps women lead a more active lifestyle.

✨This 46 yo patient was diagnosed with right breast cancer and wanted to increase her breast size with the breast reconstruction process.

✨This result was achieved in 2 surgeries. The first part of the reconstruction was performed immediately after the mastectomy. A tissue expander was placed above the chest muscles. The patient then went on to have a series of tissue expansions until we reached her desired size. A few months later, the second surgery was done: the tissue expanders were removed and replaced with silicone breast implants, and fat grafting was done to give a natural result!
