⓻ Hospice House Opens Its Doors

The opening of Hospice House marked a significant moment in the history of hospice care in Western South Dakota. The facility created a peaceful, home-like setting designed to provide comfort, privacy and support for patients and families when care at home is no longer possible.

Hospice House represents decades of dedication from caregivers, volunteers and community members who helped bring this vision to life. Today, it continues to serve as a place of comfort, compassion and connection.

Meet Sarah Plaggemeyer, RN!

Sarah is a valued part of the Monument Health Home Health and Hospice team, helping patients maintain their health, improve quality of life and receive compassionate end-of-life care in the comfort of their homes.

Q: What unique qualities or experiences do you bring to your role as a health care provider?
A: I have been a nurse since 2017, graduating from nursing school in Texarkana, Texas. After graduation, I moved to South Dakota to work for Monument Health and quickly discovered my passion for home health and hospice care. Assisting patients in their home environment to maintain their health, improve quality of life and support end-of-life care is truly my passion. I bring compassion, kindness and an ability to recognize subtle changes in my patients. I have primarily specialized in caring for the elderly, which is incredibly rewarding and allows me to support patients through many stages of health and life from the comfort of home.

Q: What are some of the key ways you create a positive experience for patients?
A: I focus on active listening, open communication and kindness while treating each patient as I would my own family. I work to be an advocate and educator for my patients, always prioritizing their needs. I lead with compassion, supportive interaction and a commitment to understanding each patient’s individual goals and health care needs.

Q: What do you enjoy most about working at Monument Health Home Health and Hospice?
A: I love our ability to meet patient needs in the comfort of their home. Often, health concerns can become more complex without early intervention. Helping patients stay safe, healthy and comfortable at home improves quality of life and allows families to remain present and involved. Our interdisciplinary team including nurses, therapists, social workers and home health aides works closely with primary care providers and specialists to support each patient’s goals. Our team is hardworking and compassionate, and I truly feel we live out Monument Health’s vision that care starts with heart.

Q: Outside of work, what do you enjoy doing? How does this help you connect with patients?
A: I enjoy spending time with my husband, our four children and my first grandson. My hobbies include attending church, fishing, outdoor activities, artwork, knitting, cooking and baking.

Q: Can you share a fun fact or something unexpected about yourself that patients may not know?
A: Becoming a grandmother has been one of my greatest joys and has given me an even deeper appreciation for family, connection and caring for others.

Meet Crystal Flinchbaugh, BSN, RN, Nurse Manager!

Crystal is a valued part of the Monument Health Home Health and Hospice team, helping support caregivers and ensuring patients and families receive compassionate, personalized care.

Q: What unique qualities or experiences do you bring to your role as a health care provider?
A: I have been a nurse for 20 years and have worked with Monument Health Home Health and Hospice for 13 years. I initially worked in the field caring for home health and hospice patients before transitioning into Case Management and later moving into the Nurse Manager role in 2019. I have always loved working with patients on a one-on-one basis. Providing care in the home setting allows care to be individualized based on each patient’s needs. I especially value the relationships built with patients and their families.

Q: What are some of the key ways you create a positive experience for patients?
A: Empathy is extremely important in Home Health and Hospice care. It is important to understand where patients are coming from and how they learn so we can best support them. I focus on listening without judgement and making sure patients know they are heard and understood.

Q: What do you enjoy most about working at Monument Health Home Health and Hospice?
A: We have a really great team. Everyone looks out for one another and works together to provide the best care possible for the patients and families we serve.

Q: Outside of work, what do you enjoy doing? How does this help you connect with patients?
A: I have started biking over the last few years and have grown to really love it. I especially enjoy mountain biking and riding my gravel bike.

Q: Can you share a fun fact or something unexpected about yourself that patients may not know?
A: I come from a musical family and played several instruments growing up. I enjoy almost every type of music, and my playlists definitely reflect that.

One of the guiding beliefs behind hospice care is that many patients feel most comfortable in familiar surroundings, surrounded by loved ones. As home health and hospice services expanded, caregivers focused on helping patients receive care wherever they call home.

This philosophy helped transform how care was delivered, allowing patients to focus on comfort, dignity and quality of life while remaining in the environment that matters most to them.

Meet Kat Yous, RN!

Kat is a valued part of the Monument Health Home Health and Hospice team, helping provide compassionate, patient-centered care while building meaningful relationships with patients and families.

Q: What unique qualities or experiences do you bring to your role as a health care provider?
A: I was born and raised in Hill City, South Dakota and attended nursing school at the University of South Dakota. I previously worked in the Monument Health Rapid City Hospital float pool, where I often floated to the Hospice House and fell in love with the care provided there. I met incredible patients, families and providers and appreciated the opportunity to build relationships with them. I chose to join the Home Health and Hospice team because it allows me to use my clinical skills while forming meaningful, long-term relationships with patients and families, which is something I missed in acute care. I bring optimism, attention to detail and a strong desire to deliver the best care possible.

Q: What are some of the key ways you create a positive experience for patients?
A: I try to visualize care from the patient’s point of view whenever possible. Our team delivers care in the patient’s space, not our own, so I always consider their reality. Patients may not be ready to discuss certain topics or receive education, and that is okay. My role is to advocate for them and meet them where they are.

Q: What do you enjoy most about working at Monument Health Home Health and Hospice?
A: I love working here because of the amazing people I have met, both patients and coworkers. The culture is kind and supportive, and there are many opportunities for continued learning. Our team is incredibly reliable and includes many experienced caregivers who have been here for years.

Q: Outside of work, what do you enjoy doing? How does this help you connect with patients?
A: Outside of work, I enjoy hiking, snowboarding and computer gaming. I am also working toward a Master’s degree in Nursing with a specialization in Informatics and E-Health. I enjoy connecting with patients through conversations about hobbies and life experiences. Many of my patients are veterans, which I relate to because my husband serves in the Air Force.

Q: Can you share a fun fact or something unexpected about yourself that patients may not know?
A: I volunteer at the Black Hills Raptor Center and help rehabilitate birds of prey.

⓸ Hospice Gains Medicare Certification

As hospice care continued to grow, a major milestone helped expand access for patients and families across the region. Hospice of the Hills became one of the first hospice programs in South Dakota to receive Medicare certification, allowing more patients to receive hospice services regardless of their ability to pay.

This certification helped hospice care reach more families while strengthening the program’s ability to provide compassionate, comprehensive support during end-of-life care.

Sharing moments from our Valentine’s Flower Delivery 💐❤️

The Cupid Crew, a partnership between Monument Health Hospice, Stillwater Hospice, Good Samaritan - St. Martin Village, and Catholic Social Services Rapid City, delivered flowers to skilled nursing facilities, the Hospice House and home hospice patients across Rapid City.

The flowers were generously donated in memory of Chic and Venie Lawrence. Venie was a former Monument Health Hospice patient and Chic was a former Stillwater Hospice patient. Their kindness made the day even more special.

Thank you to everyone who helped spread love throughout our community.

Meet Paul Lloyd, MDiv, Hospice Chaplain!

Paul is a valued part of the Monument Health Home Health and Hospice team, helping provide spiritual support, comfort and compassionate presence for patients and families.

Q: What unique qualities or experiences do you bring to your role as a health care provider?
A: I worked in local church ministry for many years. One area of ministry in the local church is helping people walk through experiences of loss and hurt in their lives. It felt like a natural transition to becoming a Hospice Chaplain. I hope through the years I have become a better listener. I have found this to be very important in walking with people through life and loss.

Q: What are some of the key ways you create a positive experience for patients?
A: I listen. At appropriate times I will ask questions but mainly just listen.

Q: What do you enjoy most about working at Monument Health Home Health and Hospice?
A: I am privileged to work with a team of professional and caring individuals. Foremost is their concern for their patients.

Q: Outside of work, what do you enjoy doing? How does this help you connect with patients?
A: Outside of work, I enjoy reading, hiking and exercising. These activities allow me to recharge and refocus.

A: I prefer grits over hashbrowns.

⓷ Volunteers Become the Heart of Hospice

Hospice care has always been built on compassion, connection and community. As hospice services expanded in Western South Dakota, volunteers quickly became a vital part of the care team. Volunteers offered companionship, emotional support and comfort to patients and families during some of life’s most meaningful moments.

From sitting bedside to simply holding a hand, volunteers helped ensure patients never felt alone. Their dedication helped shape the hospice care experience and continues to be a cornerstone of care today.
